Job Description

Job Summary
Position delivers comprehensive HIV-related medical case management and supportive services to Ryan White program-eligible individuals and families living with HIV/AIDS, with the primary goals of linkage to care, retention in care, and viral suppression. Case management includes intake, assessment, care planning, linkage and referral, ongoing monitoring, and documentation in accordance with federal and state Ryan White program standards.

Job Purpose
Position provides client-centered medical case management for people living with HIV, coordinating assessment, care planning, linkage, and follow-up care to reduce barriers, support treatment adherence, and keep clients engaged in regular HIV care.

Essential Functions
Essential functions are the primary duties/main job responsibilities that an incumbent must be able to perform, with or without accommodation. Key accountabilities include client intake, eligibility and assessment, care/service planning and coordination, continuous monitoring, retention and adherence support, documentation, data quality and compliance, and collaboration, advocacy and quality improvement.

Job Factors
Minimum Education Level Required: Bachelor’s degree in social work, psychology, human services, public health or a closely related discipline.

Minimum Experience Level Required: Minimum one year of experience in case management or a closely related social services or healthcare role. Previous direct HIV/AIDS case management experience required.

Knowledge, Skills and Abilities
• Valid Florida driver’s license with a clean driving record.
• Knowledge of HIV/AIDS, treatment options, lab markers, adherence standards and risk-reduction concepts.
• Knowledge of community resources, benefits programs, and local HIV service systems.
• Knowledge of confidentiality, HIPAA and privacy regulations and requirements related to HIV care.
• Strong assessment, care planning and service coordination skills, including psychosocial and needs assessment and development of individualized service plans.
• Excellent time management, prioritization and problem-solving skills to manage a caseload, meet deadlines and respond to crises.
• Strong written and verbal communication skills with the ability to communicate effectively with all levels of the organization and adapt messaging for the appropriate audience. Bilingual preferred.
• Ability to build supportive, professional relationships with diverse clients while maintaining boundaries, cultural humility, and non-judgmental, trauma-informed practice.
• Ability to function in a complex team environment, collaborating effectively with multidisciplinary teams to meet needs.
• Self-directed and self-motivated, demonstrated ability to meet deadlines and manage competing demands efficiently and simultaneously.
• Strong documentation and data entry skills; able to navigate Microsoft Office products and use EHR/HMIS systems, software or similar databases.
• Strong critical thinking and adaptability in a fast-changing, highly regulated environment.
• Ability to work independently with minimal supervision, meeting hard deadlines.

Physical Demands

• High mobility, prolonged standing and walking, sometimes for extended periods without breaks.
• High manual dexterity and repetitive movements using hands and fingers.
• Motor vehicle and other transportation operations between campus locations, client home/community visits and to other off-campus locations and events (Travel 50%)
• Lifting items up to 30 pounds and physical exertion.
• Frequent bending, stretching, twisting and reaching.
• Access information using a computer

Environmental Conditions
Indoors in an environmentally controlled space with exposure to environmental hazards (contaminants, hazardous equipment, and infectious disease.) Occasional work outdoors exposed to elements.
Frequent performance of position duties while standing and walking and otherwise in motion.
Frequent computer use at a workstation up to two hours or more at a time.
Frequently fast-paced work with unscheduled interruptions.
Work may require an adjusted work schedule, overtime, and evening/weekend hours in order to meet patient care demands.
Some public contact requiring business-appropriate apparel and/or occasional use of personal protective equipment.
