About This Position
We are excited to announce an opening for a Care Management Program Manager position! In this role, you will be responsible for providing supervision and management of Care Managers, Care Navigators, and RN Care Coordinators while providing indirect patient care and support for patients and families throughout their healthcare journey. This role involves developing and maintaining the care planning liaison role between hospitalist groups and IMA primary care providers to ensure the highest quality of care. The goal of the Care Management Program Manager is to improve outcomes and reduce the cost of care for IMA patients.
Key Responsibilities:
- Partner with Patient Engagement Manager to establish and implement performance initiatives that optimize patient care, efficiency and effectiveness.
- Develop and communicate qualitative and quantitative metrics for Care Managers, Care Navigators and RN Care Coordinators.
- Monitor and coach direct reports through ongoing performance reviews, engagement activities and performance improvement plans.
- Collaborate with the Learning and Development department to educate and train Care Managers, Care Navigators, and RN Care Coordinators on best practices that improve health outcomes for patients and their families, ensuring comprehensive care management.
- Work with the quality, analytics and IT teams to train staff, monitor systems and gather patient data through various platforms and health care interoperability for practice management such as electronic health record (HER), encounter notification system (ENS) alerts, and hospital portals.
- Ensure timely retrieval and storage of hospital records and discharge summaries for patients with upcoming hospital follow ups.
- Resolve escalated patient cases to provide exceptional customer service and improved patient satisfaction.
- Enforce policies and procedures to maintain expectations and best practice standards in Care Management.
- Round with hospitalist groups and RN Care Coordinators at various local and regional hospitals.
- Apply critical thinking skills to develop individualized care plans in collaboration with the healthcare team and RN Care Coordinators.
- Coordinate with the hospitalist team, hospice and the PCP to facilitate next steps in patients and family care.
- Monitor and record patient conditions, vital signs, and treatment responses to ensure the PCP has a clear picture of the patient's status.
- Provide updates to IMA providers and Office Managers through daily clinic huddles and EHR documentation for patients with emergency room or inpatient visits.
- Communicate daily with RN Care Coordinators, Care Managers and Care Navigators to discuss high acuity patient cases, identify frequent flyers and develop post discharge support plans.
- Degree in Nursing
- 5 years of experience in the hospital and/or clinic setting.
- One to three years of supervisory experience.
- CPR/BLS certification.
- Strong clinical knowledge and skills, with the ability to make critical decisions in fast-paced settings.
- Excellent communication and interpersonal skills.
- EMR Experience
- Proficient with Microsoft365 products (Team, Outlook, Excel, Word, PowerPoint, Power BI)
Preferred Requirements:
- Valid Registered Nurse (RN) license in the state of Florida
- Experience with Athena EMR
- Bilingual (English and Spanish)
