SUMMARY:
Family Service League is seeking a full-time case manager for our Children's Health Home program in Riverhead, NY. The Case Manager offers care coordination to children and young adults with mental illness and/or chronic medical conditions. The Case Manager meets with clients to develop Strength Assessments and individualized planning, as well as support them with their goal achievements.

RESPONSIBILITIES:
• The case manager will outreach and engage clients with a primary history of mental illness.
• Develop Strengths Assessment with clients.
• Develop crisis/safety planning, in collaborations with Mental Health providers.
• Develop individualized planning with the client in the areas of health, education, mental health, vocation, socialization, benefits, legal and life skills.
• Meet a minimum of 2 times per month to support the youth with their goal achievements.
• The case manager will assist in the coordination of services with providers, benefit systems and the community.
• Assist in the linkage to service providers, benefit systems and the community.
• Advocate for clients as needed.
• Help clients navigate service systems so they get the support that they need in their recovery.
• The case manager will initiate crisis intervention with linkage to appropriate mental health or health services, as necessary.
• Report all client incidents and accidents.
• Inform supervisor when other providers are not collaborating in the effort to achieve services for the client, or have plans to inappropriately discharge a client from housing.
• Follow guidelines set by Office of Mental Health and Division of Mental Hygiene SPOA unit Risk Management.
• All other duties as assigned.QUALIFICATIONS:
Bachelor’s Degree in Social Work, along with 2 years’ of related experience, or Master’s Degree and 1 year of related experience required.
Excellent interpersonal and organizational skills required.
Strong verbal and written communication skills required.
Ability to solve problems and work as part of a team required.
Proficient computer skills, including Microsoft Office, required.
Valid and clean Driver's License, and a reliable car to drive to worksites and clients homes.
PHYSICAL REQUIREMENTS:
This position requires sitting for long periods of time and travelling to conduct home visits.
