Physical Therapist- (PT) in Broken Arrow, Oklahoma at Choice Rehab

Choice Health at Home is seeking a compassionate and dedicated Physical Therapist to assess, plan, organize, and provide rehabilitative care in patients’ homes. In this role, you will help patients improve mobility, relieve pain, increase strength, restore physical function, and manage disabling conditions resulting from illness or injury.

• Perform and document comprehensive initial evaluations, analyze clinical findings, identify functional limitations, and establish a diagnosis and plan of care before beginning treatment.
• Develop and implement individualized treatment programs designed to maintain, improve, or restore physical function, alleviate pain, and prevent further dysfunction.
• Evaluate patients’ responses to treatment at various stages and modify interventions as needed to achieve the greatest possible benefit.
• Apply infection-control practices and universal precautions while providing patient care.
• Ensure continuity and quality of care through accurate, timely, and complete documentation.
• Communicate effectively with patients, families, caregivers, members of the interdisciplinary team, and other healthcare professionals.
• Educate patients and caregivers on treatment plans, therapeutic exercises, safety practices, and strategies for achieving functional goals.
• Uphold the agency’s philosophy, clinical standards, and administrative policies.
• Complete all required documentation within established company guidelines.

• Successful completion of an accredited physical therapy program.
• Current and valid state Physical Therapist license.
• Current CPR certification.
• Valid driver’s license and automobile insurance.
• At least one year of home health experience preferred.
• Experience with Homecare Homebase preferred.

• Ability to stand, walk, sit, climb, crawl, squat, and kneel for extended periods.
• Ability to lift, carry, pull, or push up to 100 pounds.
• Ability to travel to patients’ homes and safely perform the essential duties of the position.
