Job Description

1st Shift: 40 hours
Case Manager III - Bachelor's Degree $27.00 or
Case Manager III Master's Level $29.00
Scope of Service: The Case Manager position provides comprehensive case management and skill-building services to support clients in achieving greater independence and overall well-being. Conducts intake screenings and assessments to develop individualized, person-centered care plans, and collaborates with a multidisciplinary team to deliver rehabilitative support, skill development, and access to community resources. Facilitates psychoeducational groups and promotes opportunities for socialization, skill-building, and community integration. Participates in daily program operations, maintains accurate and timely documentation, and helps ensure a safe, clean, and supportive environment. Performs additional duties as assigned.

Essential Job Functions
Client Care
• Establishes and maintains positive, professional relationships with clients while adhering to ethical standards and appropriate boundaries.
• Supports the development of daily living skills by assessing needs and providing instruction through modeling, prompting, and hands-on assistance.
• Develops, implements, and monitors person-centered plans in collaboration with clients, including involvement in intake and discharge processes.
• Coordinates and supports case management needs, including referrals and access to services such as entitlements, financial management, healthcare, transportation, housing, nutrition, legal services, education, and employment.
• Assists clients in identifying barriers to achieving goals and connects them to appropriate resources and supports.
• Meets regularly with clients to review progress, mental health status, and upcoming appointments and activities.
• Supports clients in learning to use local transportation and provides transportation as needed; may transport clients in personal vehicles within community-based and shared living settings.
• Conducts routine room/apartment inspections and assists clients in maintaining clean, safe living environments, including hands-on support when necessary.
• Develops and maintains collaborative relationships with families, natural supports, and external providers, and advocates on behalf of clients as needed.
• Facilitates or co-facilitates psychoeducational groups, as appropriate.
• Completes documentation in accordance with contract and agency requirements.
• Administers breathalyzer tests and collects urinalysis samples, as applicable.
• Conducts routine program rounds to ensure safety and compliance.
• Attends and participates in staff meetings, All-Treater meetings, clinical rounds, and supervision.
• Ability to respond to crisis situations, including potential behavioral emergencies.
• Ability to travel within the community as required.
• Meets productivity standards and documentation timelines as defined by program and contract requirements.

Environmental Responsibilities
• Maintains a clean, well-organized work environment.
• Ensures all program egresses and walkways are clear to support workplace safety.
• Notifies supervisor of maintenance and/or safety concerns.
• Participates in Food Bank activities as assigned.
• Completes other designated environmental tasks as assigned.
• Issues and generates work orders as needed.
• Conducts and participates in fire and emergency evacuation drills, and reviews the emergency preparedness plan as outlined in agency policy.

Quality Assurance
• Consistently adheres to all policies, procedures, and expectations outlined in the staff handbook and program operations manual.
• Maintains respectful, cooperative, and professional conduct.
• Complies with HIPAA and secures proper release forms for external communication.
• Demonstrates cultural competence with individuals, families, providers, and staff.
• Maintains accurate electronic records in line with all agency and regulatory requirements.
• Follows Council on Accreditation of Rehabilitation Facilities (CARF) and other regulatory standards.
• Participates in agency committees when appropriate.
• Completes all required trainings per contract and agency policy.
• Attends in-services, trainings, and conferences to support ongoing professional development.
Case Manager III
• Master’s degree in a related field OR Bachelor’s degree in a related field with two (2) years of progressively responsible human services or case management experience.
• Works within community-based, residential, or acute-setting programs with increased independence.
• Participation in a rotation of on-call responsibilities, including availability for after-hours coverage as assigned.
• Coordinates care across multiple providers, systems, and community resources with limited supervisory direction.
• Independently manages caseload.
• Conducts service planning, crisis intervention, and client advocacy with greater autonomy and decision-making responsibility.
• Assists with orienting newer staff and supporting program operations, including supervision.
• Assists with staff development, training, and support as assigned.
• Serves as lead on shift in the absence of management staff.
• Demonstrates proficiency in documentation, service coordination, and client stabilization strategies.

Additional Requirements
• Experience providing services and support to individuals with mental health and/or substance use needs.
• Valid driver’s license required. Reliable vehicle required for community-based and shared-living programs. Must maintain minimum Connecticut State automobile liability insurance.
• First Aid/CPR certification or willingness to obtain certification upon hire. Employee is responsible for maintaining ongoing certification.
• Proficient reading, writing, and oral skills required, including computer proficiency and familiarity with electronic record keeping.
• Knowledge of the community support system and resources preferred.
