Job Description
World Relief seeks a Health and Wellness Case Manager to support vulnerable immigrants in building pathways to physical, emotional, and social well-being. This position provides Intensive Case Management (ICM) for individuals and families, alongside wellness-focused group programming that promotes health education and community integration. Working in a dual-capacity role, the Case Manager guides clients through complex healthcare systems, crafts tailored goal plans for long-term stability and collaborates with health providers and community partners to deliver trauma-informed, fully compliant care.
Client Services and Role Responsibilities
PC Groups & Intensive Case Management
- Oversee all incoming Preferred Communities (PC) referrals, conducting comprehensive intake assessments and client enrollments.
- Provide direct Intensive Case Management (ICM) to develop and implement targeted, individualized goal plans that foster long-term self-sufficiency.
- Maintain regular, proactive communication with clients throughout their service term to monitor progress, address emerging needs, and provide continuous support.
- Guide clients through healthcare, social service, and community systems to build their independence and confidence.
- Maintain precise, timely documentation and detailed case notes in full alignment with program standards and compliance guidelines
Interdepartmental Collaboration & Wrap-Around Care
- Work closely with the Programs Manager and program staff to streamline client service delivery across department lines.
- Partner with the Volunteer Coordinator to engage, train, and leverage volunteers or community groups for integrated client support.
- Deliver holistic, wrap-around care by connecting clients to wrap-around community services and advocacy networks.
Compliance, Data Management & Quality Assurance:
- Assure all compliance measures are strictly maintained across all PC programming and service delivery.
- Ensure data collection and outcome tracking are conducted in a timely, accurate, and effective manner for reporting and evaluation.
Administrative Responsibilities
- Actively participate in department meetings, staff development opportunities, program trainings, community activities, and organizational events
- Submit any requested data to the program manager through monthly, semi-annual, and annual reports.
- Maintain accurate and organized files and records of services provided
- Ensure continuity of services and reporting during staff transitions and in the absence of a Programs Manager
- Adhere to all World Relief policies, including but not limited to, incident reporting procedures, code of conduct, security safety and health
- Travel to and between High Point and Winston Salem office locations is a requirement for this position
- Perform other tasks and projects as assigned

- Valid driver’s license, access to reliable vehicle and ability to pass MVR standards
- Experience in case management, health education, or related fields required; experience with refugee or vulnerable populations preferred.
- Highly organized, with the ability to manage multiple cases simultaneously and prioritize tasks effectively.
- Strong understanding of the challenges faced by refugees and immigrants, including trauma-informed care and cultural sensitivity.
- Excellent verbal and written communication skills, with the ability to clearly convey information to clients, colleagues, and external partners.
- Strong interpersonal skills, including empathy, active listening, and conflict resolution.
- Fluency in English required. Bilingual in any of the following languages: Arabic, Swahili, Spanish, French, Kareni, Burmese, Pashto, Dari
- Degree in Social Work, Public Health, or similar field.
- Cross-cultural experience.

$22 - $23 an hour
