Temporary Position for Hospital Tracking Coordinator in Lincoln, Nebraska at Bluestem Health

The hospital tracking coordinator is responsible for outreach efforts to facilitate patient compliance/access; tracking patient’s ER visits and Hospital follow-up care; identifying/utilizing all appropriate available community resources; and interacting, planning and coordinating care with the providers. The hospital tracking coordinator will be responsible for facilitating care coordination services for patients who have been to the emergency department or incurred a hospital stay.

This position is Monday-Friday 8am-5pm. The position is temporary, but could turn into permanent work depending on staffing needs.

• High school Diploma, required
• Medical office or experience in a healthcare setting, preferred
• Computer skills, required; Electronic Health Record Experience preferred
• Good oral and written communication skills
