Ambulatory Care Pharmacist at Baptist Health Deaconess Madisonville – Madisonville, Kentucky

Demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient, and cost effective patient care as part of a multidisciplinary team. Collaborating on efforts to deliver patient care in a community-based care environment by expanding and enhancing pharmaceutical clinical-based services and assuring that the responsibilities outline below are carried out.

1. Minimum Education, Training, and Experience Required

• Kentucky Board of Pharmacy license in good standing.
• Bachelor's Degree in Pharmacy or Doctorate of Pharmacy required.
• Minimum of 3 years related work experience in ambulatory practice preferred, completion of ASHP accredited pharmacy residency program preferred, or Board Certification in relevant practice area required preferred.
• BLS certification preferred
• Immunization certification preferred
