Licensed Clinical Social Worker, Bilingual Spanish

Job Summary:

BMS Family Health Center is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) who is fluent in English and Spanish to join our behavioral health team. This role provides direct clinical services to adolescent and adult patients our primary care and school-based clinic settings.

The LCSW will deliver high-quality, patient-centered care through individual, family, and group therapy, while ensuring all documentation meets established mental health standards. This role also contributes to a collaborative, interdisciplinary care environment.

Essential Functions

  • Collaborate with the Director of Behavioral Health to support clinical goals related to adolescent and adult mental health services.
  • Conduct psycho-social assessments and patient needs analysis to develop treatment plans and appropriate referrals (e.g., psychiatric evaluation, hospitalization, in-home services).
  • Partner with clinical staff to implement and complete screenings for depression, anxiety, substance use, alcohol, and tobacco.
  • Provide individual, family, and group psychotherapy based on comprehensive assessments and treatment plans.
  • Coordinate therapy sessions for patients and families.
  • Deliver crisis counseling and intervention services as needed.
  • Communicate and coordinate care with ACS, case managers, and other collateral contacts.
  • Complete psychosocial assessments and develop individualized treatment plans, updating them regularly.
  • Maintain accurate and timely documentation in the Electronic Health Record (EHR), including closing progress notes within 48 hours.
  • Share relevant information with care partners to support optimal patient outcomes.
Requirements:

Required Qualifications

  • Master’s degree in social work from an accredited program
  • Valid New York State LCSW license
  • 5 years of social work experience
  • Completion of Mandated Reporter Training
  • Current CPR and Infection Control certifications
  • Strong organizational, time management, and planning skills
  • Excellent written and verbal communication skills.
  • Bilingual in English and Spanish (advanced or native Spanish language speaker).
  • Strong knowledge of child and adolescent development.
  • Ability to build positive, trusting relationships with youth and families.
  • Excellent clinical documentation and professional writing skills.
  • Proficiency with Microsoft Office Suite and Electronic Health Records.
  • Effective communication, active listening, and interpersonal skills.
  • Ability to identify high-risk psychosocial factors affecting patient health.
  • Demonstrated ability to work effectively in a fast-paced, interdisciplinary team environment.
  • Sound judgment and ability to make timely decisions in high-pressure situations.
  • Commitment to maintain confidentiality and ethical standards.
  • Participation in recredentialing and annual training requirements.
