BMS Family Health Center is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our behavioral health team. This role provides direct clinical services to adolescent and adult patients our primary care and school-based clinic settings.
The LCSW will deliver high-quality, patient-centered care through individual, family, and group therapy, while ensuring all documentation meets established mental health standards. This role also contributes to a collaborative, interdisciplinary care environment.
Fluency in English and Spanish is a plus!
ESSENTIAL FUNCTIONS
- Collaborate with the Director of Behavioral Health to support clinical goals related to adolescent and adult mental health services.
- Conduct psycho-social assessments and patient needs analysis to develop treatment plans and appropriate referrals (e.g., psychiatric evaluation, hospitalization, in-home services).
- Partner with clinical staff to implement and complete screenings for depression, anxiety, substance use, alcohol, and tobacco.
- Provide individual, family, and group psychotherapy based on comprehensive assessments and treatment plans.
- Coordinate therapy sessions for patients and families.
- Deliver crisis counseling and intervention services as needed.
- Communicate and coordinate care with ACS, case managers, and other collateral contacts.
- Complete psychosocial assessments and develop individualized treatment plans, updating them regularly.
- Maintain accurate and timely documentation in the Electronic Health Record (EHR), including closing progress notes within 48 hours.
- Share relevant information with care partners to support optimal patient outcomes.
REQUIRED QUALIFICATIONS
- Master’s degree in social work from an accredited program
- Valid New York State LCSW license
- 5 or more years of social work experience
- Bilingual in English and Spanish (advanced or native Spanish language speaker), preferred.
- Completion of Mandated Reporter Training
- Current CPR and Infection Control certifications
- Strong organizational, time management, and planning skills
- Excellent written and verbal communication skills.
- Strong knowledge of child and adolescent development.
- Ability to build positive, trusting relationships with youth and families.
- Excellent clinical documentation and professional writing skills.
- Proficiency with Microsoft Office Suite and Electronic Health Records.
- Effective communication, active listening, and interpersonal skills.
- Ability to identify high-risk psychosocial factors affecting patient health.
- Demonstrated ability to work effectively in a fast-paced, interdisciplinary team environment.
- Sound judgment and ability to make timely decisions in high-pressure situations.
- Commitment to maintain confidentiality and ethical standards.
- Participation in recredentialing and annual training requirements.
