Job Description:

The Nurse Care Manager provides support to targeted patients by coordinating care with patient hospital observation and inpatient discharges. The Care Manager will ensure the provision of quality healthcare services in a cost-efficient manner through daily planning, coordination, education, and monitoring of care. The Care Manager will track gaps in care and relevant outcomes measures for the patients to ensure complete and effective care.

To qualify you must have:

• Current RN license for the State of Indiana required.
• Minimum of 2 years in care management preferred.
• Experience with relevant systems (e.g. EMRs, disease registries).
• Knowledgeable about population health management
• Requires ability to use logical problem-solving skills and make independent decisions based on the nursing process and the CIN policies/ procedures.
• Requires motivation and critical thinking skills.
• Must have effective judgment and/ or action during critical situations.
• Must provide compassionate assistance to people with medical or mental health needs.
