The Comprehensive Care Manager, under the direction of the Director of Care Coordination, assumes responsibility and accountability for facilitation and day-to-day direction of the work of the department/ assigned area of work. The Manager assists in the provision of cost-effective quality service through collaboratively working with and supporting the staff of the Comprehensive Care Program. The Manager demonstrates strong organizational and planning skills and is able to work collaboratively with managers, administration, staff, and physicians. The Comprehensive Care Manager is responsible for the day-to-day planning, organization, management, and evaluation of care coordination activities for the Comprehensive Care Program and other programs/services as directed. Care Coordination focuses on five primary areas: population management, patient self-management support, transitions in care, resource connection, and appropriate resource utilization. The Manager creates a working environment to facilitate and achieve operational excellence and regulatory compliance related to care coordination efforts.
Duties & Responsibilities
- Provides management and oversight for the daily operations of care coordination and related activities within the AnMed Health Comprehensive Care Program.
- Identifies opportunities for change and/or innovation in the execution and /or design of assigned areas.
- Designs and/or supports a process for communicating and collaborating within the multidisciplinary team involved in the care of patients within the program to effect appropriate care coordination.
- Maintains an operational framework to focus the activities of the care coordination team on the achievement of optimal outcomes, resource utilization, clinical expertise, and improvement strategies.
- Supports efficient resource utilization and optimizes clinical outcomes for assigned patients.
- Supports the identification of patients who are high risk for self-management, clinical or social resource needs, and readmissions and designs the most appropriate plan of care and/or needed services in conjunction with the multidisciplinary team to optimize patient outcome and patient self-management goals.
- Oversees the care planning process for assigned client /patient populations.
- Participates in the development of departmental and organizational goals and implements action plans to accomplish established goals.
- Monitors department activity towards achieving these goals.
- Recognizes department deficiencies and develops innovation and/or action plans.
- Demonstrates effective communication skills and fosters an open environment within the department and organization.
- Builds teamwork within the Care Coordination model across functions and staff.
- Manages staff concerns effectively utilizing appropriate resources and channels of authority.
- Adapts quickly to changes in process, criteria and/or focus, and fulfills duties responsibly with little supervision.
- Stays abreast of changing clinical trends and criteria related to clinical documentation activities.
- Recommends process improvements of workflow and/or policies and procedures as necessary.
- Notifies leadership in a timely manner of issues to be addressed.
- Demonstrates effective problem-solving skills.
- Monitors department performance to ensure operational goals and targets are met as well as compliance with established policies, procedures, and external standards.
- Responsible for needed policy and procedure development for care coordination activities.
- Identifies education needs for staff and is responsible for ensuring educational goals and resources are obtained in a timely manner.
- Assumes responsibility for professional development by participating in workshops, conferences, and/or in-services and maintains appropriate records of participation.
- Maintains established departmental and organizational policies and procedures, objectives, and safety standards.
- Maintains professional integrity and confidentiality of information in accordance with AnMed Health policies and procedures, state, and federal requirements and applicable laws.
- Functions as a Case Manager as needed.
- Performs other duties as assigned.
Qualifications
- Bachelor’s in nursing.
- Minimum five years of clinical experience with minimum of three years’ experience with Case Management, Quality Management, Community or Population Health Management programs with minimum of two years management experience.
- Current SC license as RN.
- Must possess excellent interpersonal communication, critical thinking and negotiation skills, problem solving skills, strong organizational and time management skills, and the ability to work independently and as a collaborative team member.
- Ability to meet deadlines and schedules is required.
- Knowledge of care management and transition of care models/systems.
Preferred Qualifications
- Accredited Case Manager (ACM) or similar certification.
