Admitting Registered Nurse - Hospice in Ventura County, California at Livingston Memorial VNA

Livingston Memorial is looking for a Full Time Admitting Registered Nurse, 40 hours a week to support our Hospice team. Schedule is Thursday-Monday 12pm to 8:30pm

Position Summary:

This position will specialize in the initial evaluation and admission of patients to hospice care. They assess patients' medical conditions, explain hospice services, and guide them and their families through the admission process. They work as part of an interdisciplinary team to create individualized care plans focused on comfort and addressing both medical and emotional needs

What You’ll Do:

• Conduct and document the nursing component of the comprehensive assessment on admission.
• Determine program eligibility, needed services, and verifies payer sources.
• Perform thorough physical and psychological assessments to determine patient needs and appropriateness for hospice care.
• Provide information about hospice care, its philosophy, and services to patients and their families.
• Guides patients and families through the admission process, including obtaining informed consent and completing necessary paperwork.
• Developing initial care plans in collaboration with the interdisciplinary team, addressing medication needs, equipment requirements, and other aspects of care.
• Addressing immediate concerns and providing initial symptom management as needed during the admission process.
• Ensures clinical documentation is complete, accurate, professional, and submitted to Livingston by end of day.

What We’re Looking For:

• Valid California Registered Nurse License
• Current Basic Life Support Certification
• One-year professional nursing experience.
• Bilingual in Spanish preferred.
• Valid Driver License and auto insurance.
• Knowledge of computer, proficient with Microsoft Word and Excel.
• Good verbal and written communication skills.
• High level of attention to details.
• Able to work independently, take ownership and responsibility of tasks.
• Strong professional ethics, discretion and judgment.
• Good organizational skills.
