Advanced Hospice is seeking a compassionate and dedicated Full-Time Medical Social Worker (MSW/LCSW) to join our caring team. In this meaningful role, you will provide psychosocial support, resource coordination, counseling, and guidance to patients and families as they navigate the challenges of living with a life-limiting illness.

• Passionate about patient care and strongly committed to improving the quality of life for patients and their families
• Independent and collaborative, with the ability to work effectively both autonomously and as part of an interdisciplinary team
• Skilled in psychosocial assessment, with the ability to develop meaningful interventions and patient-centered goals
• Proactive and resourceful, using knowledge, experience, and community resources to address patient and family needs
• Compassionate and professional, with a genuine desire to support individuals and families during an important and challenging time in their lives

Qualifications

• MSW or LCSW required
• Current, valid driver's license
• Proof of current auto insurance
• Reliable transportation
• Previous hospice experience preferred

Schedule

Monday–Friday | Daylight Hours | Full Time
