Job Title
Clinic Medication Refill RN
Job Summary
The Clinic Medication Refill Registered Nurse (RN) is responsible for the timely, accurate, and protocol-driven processing of patient medication refill requests across the clinic. The primary duty of this role is to manage medication refills in accordance with established clinic refill protocols, ensuring patient safety, regulatory compliance, and continuity of care.
In addition to refill management, the Medication Refill RN provides secondary support for patient triage via phone, responding to patient care needs, providing clinical leadership to the clinic care team, coordinates patient care services, provides patient care and ensures timely access to care. Uses standardized, medical staff approved protocols to provide patient care, telephone triage, advice and health education for ancillary staff, clinic patients, and callers to the clinic. This position provides age-specific and developmentally appropriate nursing care and triage for neonate, infant, pediatric, adolescent, adult, and geriatric patients.

There is an opportunity for this position to be remote, based on departmental needs and approval.
Compensation
We are committed to providing wage ranges for all open positions. Please note that the specific compensation for this role will be determined based on experience, education and internal equity considerations. The hourly wage range for this position is $41.06—$57.96. This range reflects the base wage for this position.
Key Accountabilities
Manages medication refill requests to ensure safe, timely, and appropriate continuation of patient care.
  • Reviews medication refill requests from patients, pharmacies, and care team members through the EHR, phone calls, and fax
  • Verifies medication details, allergies, diagnoses, recent visits, and required monitoring per clinic policy and standing orders.
  • Determines refill eligibility based on established protocols, standing orders, and provider guidance.
  • Processes medication refills independently when criteria are met, escalating to provider when outside RN scope or protocol.
  • Communicates clearly with patients and pharmacies regarding refill status, need for appointment, labs, or provider review.
  • Uses SBAR format to route refill requests or concerns to providers when appropriate.
  • Ensures accurate, timely, and complete documentation of all refill-related actions in the electronic medical record.
Triages patient care phone calls and walk-ins to clinic.
  • Assess patients’ needs with triage procedure book on the phone to determine appropriate level of care.
  • As appropriate, schedules appointments with providers.
  • Using SBAR format, document in EMT patient chart.
  • Triage walk-in patients for the level of care needed.
  • Develops teaching plans for patients.
  • Refills medications via standing order when appropriate.

Performs duties as clinic RN as needed

  • Assesses and intervenes with appropriate care.
  • Provides accurate and timely documentation.
  • Provides safe and effective nursing care and support to the provider team.
  • Delegates tasks to unlicensed care team as needed to implement care.
Provides clinical leadership to clinic team to ensure age-specific, developmentally and culturally appropriate care is provided to patients.
  • Ensures quality clinical service delivery at clinic.
  • Problem solves with/for clinic team, removing barriers and facilitating collaboration of care.
  • Participates in quality improvement work and development of protocols, procedures and policies. Provides health education to clinic team supporting growth and enhancing team performance.
  • Serves as a role model, upholding high standards and consistently demonstrating the organization’s values and excellent customer service.
  • Promotes a safe environment.
Collaborates with health care team and patient/families for patient-focused care
  • Collaborates with internal and external resources.
  • Acts as a primary resource for patients.
  • Works in collaboration with the provider team for patient-focused care and achievement of patient outcomes based on diagnosis and prognosis
Provides comprehensive care coordination and utilization of resources to meet the patient’s health care needs.
  • Identifies and resolves barriers to access care.
  • Develops care plans and interventions, implements the plans and evaluates progress. Collects, prioritizes and synthesizes comprehensive information and data pertinent to patients’ health needs.
  • Coordinates patient care for assigned practice team.
Required Qualifications
  • Current and unrestricted licensure as a RN in the State of Minnesota.
  • Current BLS must be enrolled in NH+C RQI program and complete the requirements within 90 days of enrollment. Must remain current (complete quarterly assignments prior to the end of each calendar quarter)
  • Three years of clinical experience in a clinical/patient care environment.
  • Previous clinical leadership experience.
  • Previous triage experience.
  • Must be able to speak, read, and write English proficiently to ensure safe and effective communication in a healthcare environment.
  • Excellent customer service skills.
  • Demonstrated critical thinking and assessment skills.
  • Strong organization, problem solving and decision-making skills.
  • Ability to learn and use electronic health records and other computer programs
Preferred Qualifications
  • Specialty experience in ER or Urgent Care.
  • Care coordination experience in a clinic environment.
Schedule
  • 80 hours per two-week pay period
  • Monday through Friday, 8:30 a.m. – 5:00 p.m.
