As a Weekend Home Health Nurse, you'll provide skilled nursing services during scheduled home visits.

Responsibilities include:

• Perform skilled nursing visits in patients' homes.

• Provide wound care, dressing changes, and wound assessments within your scope of practice.

• Administer prescribed medications and treatments.

• Assist with diabetes management, including blood glucose monitoring, insulin administration, and patient education.

• Monitor patient conditions and communicate significant changes to the appropriate clinical team members.

• Assess patient needs and implement established plans of care within your professional scope.

• Educate patients and caregivers on medications, chronic disease management, and preventive care.

• Maintain accurate, timely clinical documentation.

• Collaborate with physicians, clinical supervisors, and interdisciplinary team members.

• Promote patient safety, dignity, comfort, and independence.

Qualifications:

• Active Massachusetts RN or LPN license in good standing.

• Strong clinical assessment and communication skills appropriate to your license.

• Experience with medication administration, wound care, and chronic disease management.

• Ability to work independently while maintaining effective communication with the clinical team.

• Strong documentation and time-management skills.

• Reliable transportation and willingness to travel to patients' homes.

• Home health or community nursing experience preferred.
