Community Health Centers of America (CHCA) is a Federally Qualified Health Center Look-Alike (FQHC-LAL) committed to providing high-quality, patient-centered healthcare regardless of a patient's ability to pay. Through our Skilled Nursing Facility (SNF) Program, CHCA delivers comprehensive primary care and specialty services to medically complex patients residing in post-acute and long-term care settings throughout Northern California.

CHCA is seeking a full-time SNF Clinical Operations Coordinator to join our Skilled Nursing Facility Practice team at our Fair Oaks office. The SNF Clinical Operations Coordinator serves as an integral member of the clinical operations team, supporting the delivery of timely, coordinated, and compliant patient care across multiple SNFs. This position works closely with providers, facility leadership, credentialing, quality, and operational teams to coordinate provider readiness, patient onboarding, clinical workflows, documentation compliance, and quality initiatives that directly support patient access and continuity of care. The regular work schedule will be Monday - Friday.

This position is ideal for an organized, detail-oriented professional who enjoys improving processes, coordinating multiple priorities, and supporting multidisciplinary healthcare teams. Additionally, this position is intended to grow alongside CHCA's expanding SNF Program and offers opportunities for advancement into Clinical Operations Management or Program Management based on performance and organizational needs.

Key Responsibilities:

Clinical Operations

  • Coordinate daily provider operations across multiple SNF locations to support uninterrupted patient care.
  • Facilitate provider scheduling and coverage to maintain continuity of patient services.
  • Coordinate onboarding of new providers, including access to clinical systems, facility orientation, and operational readiness.
  • Run daily admission and census reports to support timely patient assignments and provider awareness.
  • Coordinate patient panel distribution and provider assignments based on admissions, discharges, and facility needs.
  • Assist with implementation of new clinical programs, workflows, and service lines.

Care Coordination & Workflow Support

  • Serve as the primary operational liaison between providers, facility leadership, and internal CHCA departments.
  • Identify workflow barriers affecting patient care and collaborate with leadership to implement solutions.
  • Coordinate provider communication regarding admissions, discharges, transfers, specialty services, and operational updates.
  • Support efficient transitions of care through accurate communication and documentation processes.

Clinical Documentation & Compliance

  • Monitor provider documentation completion to support regulatory compliance and continuity of care.
  • Track outstanding documentation and facilitate timely completion.
  • Assist with implementation of documentation standards and workflow improvements.
  • Maintain operational compliance trackers related to provider readiness, documentation, quality initiatives, and regulatory requirements.
  • Support compliance with Title 22, CMS, FQHC, and organizational documentation standards.

Quality & Performance

  • Participate in Quality Assurance (QA), Quality Improvement (QI), and Quality Management (QM) initiatives.
  • Prepare operational reports and dashboards to support clinical leadership.
  • Assist with provider productivity monitoring and operational performance metrics.
  • Support ongoing quality improvement projects designed to improve patient outcomes, efficiency, and regulatory compliance.

Provider Support

  • Coordinate provider recruitment logistics, interviews, onboarding, and orientation.
  • Maintain provider schedules, coverage calendars, and operational resources.
  • Coordinate access to EHR systems, facility credentials, and required documentation.
  • Assist providers with operational questions, workflow support, and problem resolution.

Requirements:

Qualifications:

  • Associate degree or equivalent healthcare experience
  • Two years of experience in healthcare operations, medical office administration, care coordination, or clinical support
  • Excellent organizational, project management, and communication skills
  • Strong attention to detail and ability to manage multiple priorities
  • Proficiency with Microsoft Office Suite (Excel, Teams, Outlook, Word, PowerPoint)
  • Ability to work independently while collaborating effectively with multidisciplinary teams
  • Experience in Skilled Nursing Facilities, post-acute care, home health, hospice, or primary care preferred
  • Experience using PointClickCare, eClinicalWorks, or other electronic health record systems preferred
  • Medical Assistant, Health Information Management, Healthcare Administration, or similar certification preferred
  • Knowledge of CMS, Title 22, HIPAA, and healthcare documentation requirements preferred
  • Experience supporting quality improvement or clinical operations initiatives preferred
