BASIC FUNCTION:

The licensed practical nurse is a graduate of an approved program of practical nursing licensed in the state of Ohio and is authorized to practice nursing under the direction of an RN.

JOB DUTIES:

1. Correctly demonstrates clinical skills necessary to provide safe nursing care.
2. Organizes and sets priorities to provide optimum continuous care.
3. Utilizes bedside charting by documenting care at time of performance.
4. Documentation is clear, concise, and legible.
5. Reports abnormal patient assessment to RN in a timely manner.
6. Responds appropriately in an emergency.
7. Participates and documents patient/family teaching.
8. Participates in discharge planning process.
9. Responsible for checking e-mails and online learning assignments (Relias) on a regular basis.
10. Other duties as assigned.
11. If designated to Senior Care Unit:
a) Understands restraint/seclusion policy, procedure, assessments, time frames, criteria for
discontinuation and documentation.
b) Understands the development of the Individual Treatment Plan, treatment team meeting time frames, evaluation, and documentation.
c) Is able to facilitate and participate in group therapy activities.
d) Provides input to Individual Treatment Plan.
e) Assists with group therapy, occupation, and recreational activities.

AGE APPROPRIATE CARE:

Colleague has been instructed in providing customer service and on interacting with age groups as related to the department's general client population: including use of appropriate interventions, communication and skills to match the age and/or abilities of the patient/family.

EDUCATION, TRAINING AND EXPERIENCE:

1. Graduate of an accredited school of practical nursing, with validation of medication administration certification.
2. Currently licensed by the Ohio State Board of Nursing to practice practical nursing.
3. Has been actively employed in hospital-based nursing within the past 5 years or show a certificate of completion from a state-accredited L.P.N. refresher course in the past year or is a recent graduate of an accredited school of practical nursing.
4. Successful completion of the Licensed Practical Nurse Orientation Program at Alliance Community Hospital.
5. Maintains current B.L.S. certification and required C.N.E.’s.
6. Shows evidence of self-growth through participation in continuing education.
7. Successful completion of any additional educational requirements outlined as conditions of employment at time of hire.
8. Must be computer literate to complete task(s) as required. Such as MediTech (Hospital Support Software), Halogen (performance appraisal system) Relias (education system), ability to retrieve e-mails, complete online enrollment for health insurance/benefits and any other programs as assigned.
9. Senior Care Unit – Crisis Intervention and De-Escalation (CIDE) certified within 3 months.

MACHINES, TOOLS, EQUIPMENT USED:

1. Must be knowledgeable and competent in the use of all equipment used within the department.

PHYSICAL REQUIREMENTS:

1. Able to stand/walk for up to full scheduled shift per day.
1. Able to bend to reach items on floor or bedside level.
2. Able to reach overhead.
3. Able to reach over a bed far enough to give care and turn patients.
4. Able to push or pull patients or necessary equipment (i.e., wheelchair, EKG machine, code cart).
5. Able to lift 50 lbs. intermittently, unassisted.
6. Able to move/transfer and ambulate patients unassisted and/or assisted as necessary using good body mechanics or appropriate techniques (i.e., slide board, total lift).
7. Able to lift, hold, and/or support a patient’s extremity or trunk for a period of time, during a procedure.
8. Able to climb onto step stool and to apply a minimum of 25-pound pressure/force for compressions during CPR.
9. Able to wear personal protective equipment for an extended period of time, as necessary, (i.e., mask, goggles, etc.).
10. Able to turn and twist as necessary.
11. Able to squat, kneel, or crawl as necessary.
12. Ability to see clearly and able to distinguish appropriate rhythms from a minimum distance of 6 feet.
13. Able to hear normal speaking voices.
14. Able to communicate verbally and over the telephone in a clear speech.
15. Able to write clearly.
