Nurse Supervisor - Hospital/Admission/Referral & Intake

At Unity, we believe exceptional patient care begins with exceptional people. We are seeking a compassionate, experienced Registered Nurse who enjoys balancing hands-on patient care with leadership. In this role, you will spend approximately half your time caring for patients and half leading and supporting nursing staff across our Hospital, Admissions, and Referral & Intake teams.

Region/Location: Greater Green Bay area

Schedule: Monday-Friday, day shift, with an every 3rd weekend rotation, on-call responsibilities approximately 10-15 times per year, and coverage for 1-2 holidays annually.

Job Duties:

Staff Supervision

· Provide ongoing coaching, mentorship, and clinical guidance to assigned staff, addressing performance and behavioral concerns while escalating significant issues to the appropriate leader.

· Conduct regular leadership rounding and team check-ins to build relationships, reinforce expectations, and support employee engagement.

· Foster a respectful, collaborative work environment by addressing staff conflicts promptly and promoting accountability and professionalism.

· Support the onboarding and successful integration of new team members by providing role clarity, guidance, and mentorship.

· Partner with Educational Services to identify learning needs, support staff development, and conduct annual EMPOWER meetings in accordance with organizational guidelines.

Team Operations & Development Leadership

· Lead daily huddles and weekly POD meetings to promote communication, operational readiness, and team alignment.

· Serve as the primary clinical resource for frontline staff, providing real-time guidance and coordinating with Education leadership as needed.

· Foster a positive team culture that promotes collaboration, accountability, engagement, recognition, and continuous improvement.

· Communicate productivity, performance, and workflow trends while implementing approved process improvements and reinforcing consistent practices.

· Identify workflow barriers, safety concerns, and operational opportunities by gathering staff feedback and collaborating with leadership, Education, and Quality teams to improve patient care and team effectiveness.

Direct Patient Care

· Deliver direct patient care in accordance with the Registered Nurse (RN) job description, licensure requirements, professional standards, and organizational policies.

· Maintain current clinical competencies while collaborating with interdisciplinary team members to provide coordinated, patient-centered care.

· Complete accurate, timely, and compliant clinical documentation and communicate clinical trends, risks, or resource needs to the Operations Manager or Educator.

· Participate in case conferences, interdisciplinary meetings, and care planning discussions to support quality patient outcomes.

· Provide patient care coverage during periods of high census, staffing shortages, or unplanned absences.

· Participate in required weekend, holiday, and on-call rotations in accordance with organizational policy.

Knowledge, Skills, and Abilities:

  • Computer proficiency (MS Office – Word, Excel, Electronic Medical Record, Scheduling System, and Outlook).
  • Demonstrated adaptability in a fast-paced healthcare environment.
  • Highly motivated and organized with a strong attention to detail and analytical skills to help solve problems.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to maintain confidentiality and discretion in business relationships and exercise sound business judgment.
  • Ability to keep up to date with policies, evidence-based best practices, and regulations.
  • Ability to prioritize and manage multiple tasks and capable of dealing with deadline pressures.
  • Comply with all applicable local, state, and federal laws and regulations as well as policies and procedures of Unity.
  • Satisfy mandatory training requirements regarding local, state, and federal regulations and/or Unity’s policies and procedures.
  • Perform other job-related duties as assigned.
  • Ability to adjust to frequently changing demands.

Requirements:

Minimum Qualifications & Certification/Licensure:

  1. Associate degree in Nursing required, bachelor’s or master’s degree preferred.
  2. RN license in the state of Wisconsin required (RN must practice under the law and regulations governing nursing in the state of Wisconsin).
  3. Minimum 3 years of clinical nursing experience.
  4. Minimum 3 years of leadership experience in home health, hospice, or facility/community-based care.
  5. Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage.
