Under the supervision of the PACE Medical Director, the ARNP provides care and treatment of participants served by the PACE program’s interdisciplinary team (IDT). S/he provides direct medical care as needed to participants in the home, the PACE center, and in other multiple health care settings.

Position Summary
Under the supervision of the PACE Medical Director, the ARNP provides care and treatment of participants served by the PACE program’s interdisciplinary team (IDT). S/he provides direct medical care as needed to participants in the home, the PACE center, and in other multiple health care settings.
How You’ll Make an Impact
Administrative:
- Provide informal clinical education and guidance to clinical staff. Collaborate with team to assure quality care is given.
- Actively participate in the IDT. Assist in developing a plan of care for each patient/family with the coordination of the patient's physician(s) and other members of the 1DT.
- Review the active medical care and/or palliative care and provide appropriate professional guidance as needed and in compliance with established procedures.
- Attend and participate in physicians' meetings, team meetings, and in-services as required and other related activities.
- Manage compliance with medical records procedures, including accurate and detailed documentation, timely dictations, and signatures as required.
- Participate in 24 hour coverage as required to maintain 24 hour 7 day/week coverage for the hospice program.
- Make visits on participants in the inpatient units as assigned, coordinating with other members of the IDT to provide quality care for the patients and their families.
- Participate in the quality process. Audit charts. Serve on committees and/or focus groups.
- Provide medical direction and guidance to the program and other staff, volunteers, to assure the maintenance of quality of care for the patients and families.
- Communicate and work positively and effectively between teams and departments, patients, families, caregivers and other professionals.
- Participate in community and professional activities for Hope.
- Work cooperatively with the PACE Program Director to ensure quality care delivery to all participants.
- Work cooperatively with clinical staff.
- Serve on appropriate organizational and corporate committees.
- Lead specific projects as appropriate and directed by the PACE Medical Director.
- Assist in the development of written guidelines and protocols as appropriate for the PACE program.
- Participate in medical reviews as needed and directed by the PACE Medical Director.
- Other duties as requested by the PACE Medical Director.
Clinical:
- Implement care protocols for the program.
- Perform comprehensive history and physicals on new referrals.
- Provide clinical expertise and participate in IDT decisions regarding diagnosis and medical management of participant needs, assuring that the highest quality of medical care is provided and that continuity and comprehensiveness of care is maintained.
- Provide suggestions to the IDT that foster high quality cost effective care for all participants.
- Participate in decisions regarding appropriateness of admissions, as necessary.
- Act as a consultant to other members of the interdisciplinary team as needed.
- Liaison with participant's referring/primary.
- Serve as the primary care physician for participants enrolled in the PACE program.
- Establish and implement a clinic schedule to provide direct participant care/medical services to participants enrolled in the PACE program.
- Establish and implement a preventive care program as appropriate for participants enrolled in the PACE program.
- Manage and round on PACE participants admitted to long term care facilities or acute in participant facilities.
- Conduct family meetings as appropriate and as needed for PACE enrollees.
- Assist in securing medical records to better determine appropriate medical care for participants.
- Provide coverage in the event of absence of other medical staff.
What You’ll Bring
- Advanced Nursing degree (Master's level) from an accredited nursing school (specialty in Adult Health, Primary Care or Geriatrics preferred).
- License to practice Advanced Nursing in the State of Florida.
- Credentialed through ANCC.
- Knowledge of the principles and practice of primary medical care.
- Ability to enhance, by association with the PACE program, the program’s visibility and reputation with practicing physicians and area hospitals.
- Ability to work collaboratively with PACE staff and volunteers as part of the IDT.
- Excellent communication skills with participants/families/staff and community contacts.
- At least three years' experience as an ARNP.
- Must have at least one year experience working with a frail/elderly population.
- Valid Florida driver's license and automobile insurance as per company policy.
- Ability to make home/residence visits.

- Florida License to practice Advanced Nursing.
