Job Description
SUMMARY: The Intake Specialist provides patient intake and service coordination including information, screening, support, and navigation of incoming referrals.
DUTIES AND RESPONSIBILITIES:

• Provide excellent customer service to patients.
• Be attentive to the customer to fully understand their situation.
• Document call information according to Standard Operating Procedures
• Work with referral sources to gather required health information for a streamlined transition to the program.
• Process all aspects of new consults, including booking within 24 hours, calling patient, notifying referral source and inputting patient demographics.
• Complete Referral data reporting weekly on consults and other metrics as requested.
• Monitor wait times of new consults, notifying manager immediately if wait times exceed 2 weeks.
• Service co-ordination and follow up.
• Demonstrate a strong commitment to quality care and service delivery.
• Other duties as assigned.
REQUIRED QUALIFICATIONS:
• Post-secondary education preferred
• Experience in the healthcare or medical industry preferred
• First Aid Certification preferred
• Strong oral and written communications skills.
• Strong organizational, analytical, task and time management skills.
• Proficient in Microsoft Office applications.
