Job Description

Location: Multiple Locations (Northern, Southern or Western suburbs

The Role

This is a part-time, multi-site clinical position tailored for a Nurse Practitioner who excels in geriatric assessment and preventative care. In this role, you will lead Annual Wellness Visits (AWVs) in your region, ensuring our geriatric population receives comprehensive, high-quality screenings and personalized prevention plans.

You will:

  • Conduct comprehensive Annual Wellness Visits at various Assisted Living, Independent Living, and Long-term Care facilities.
  • Perform detailed cognitive screenings, functional assessments, and fall risk evaluations.
  • Lead medication reconciliation and facilitate meaningful Goals-of-Care discussions.
  • Develop and update patient-centered Care Plans based on AWV findings to address chronic conditions and preventative health goals.
  • Identify and enroll eligible patients into Chronic Care Management (CCM) programs, ensuring they receive the continuous support necessary for complex health needs.
  • Ensure thorough documentation and clinical coding to capture all wellness measures

Ideal Candidate

  • Board-certified NP
  • Interest in geriatrics (experience preferred but not required)
  • Comfortable in multiple care settings
  • Strong communicator with patients and families
  • Detail-oriented and thoughtful clinician
  • Values mentorship and team-based practice
New graduates with strong clinical rotations in geriatrics or internal medicine are encouraged to apply
Schedule & Structure
  • Salary: $75 per hour/completed visit
  • Part-Time: 16-32 hours per week
  • Location: Multi-site rounding in your region (Northern, Southern or Western suburbs) .
  • No Call/Weekends: Focused exclusively on scheduled wellness assessments.
