Position Summary
The Intake Care Coordinator is a non-clinical, centralized role responsible for managing the intake workflow for both home health and hospice services from referral receipt through admission handoff. This position serves as the first point of contact for referral sources and patients, ensuring timely processing, accurate documentation, insurance verification, and efficient coordination with clinical teams to support seamless admissions and optimal conversion.
The role does not provide clinical care or make clinical decisions.
Core Responsibilities:
Referral Intake & Coordination
- Manage home health and hospice referrals from receipt through clinical handoff.
- Process admissions in HCHB and ensure timely Start of Care.
- Support intake operations during high-volume periods.
Insurance Verification & Authorizations
- Verify patient eligibility, benefits, and coverage requirements.
- Obtain required authorizations prior to admission.
- Resolve insurance issues to avoid delays in care.
Documentation & Data Management
- Enter and maintain accurate patient, referral, and insurance information in HCHB.
- Ensure intake documentation is complete, compliant, and uploaded timely.
- Maintain HIPAA-compliant records.
Communication & Referral Partner Support
- Serve as a primary contact for referral sources, facilities, physicians, and case managers.
- Provide timely referral status updates and coordinate communication with clinical teams.
- Maintain intake communication threads and supporting documentation.
Admission & Scheduling Support
- Coordinate admissions and Start of Care with clinical and scheduling teams.
- Arrange DME, transportation, and other admission-related logistics as needed.
- Support census tracking and operational reporting.
Compliance & Quality
- Follow company policies, HIPAA requirements, and regulatory standards.
- Support quality initiatives, audits, and continuous process improvement.
- Help reduce admission barriers and improve referral-to-admission conversion rates.
Position Qualifications:
Required
• Minimum 2 years of experience in healthcare intake, coordination, scheduling, or administrative operations
• Experience in a regulated healthcare environment (home health, hospice, or similar)
• Strong organizational, multitasking, and time management skills
• Proficiency in EMR systems (Homecare Homebase preferred) and Microsoft Office
• Excellent communication, phone etiquette, and customer service skills
• Ability to work in a fast-paced, high-volume, centralized environment
• Demonstrated professionalism and ability to maintain confidentiality
• Ability to work independently as well as on a team
Preferred
• Experience supporting both home health and hospice operations
• Familiarity with payer requirements, authorizations, and eligibility processes
• Experience in centralized or shared services models

Pay: $26-$29 per hour depending on experience
