Physical Therapist (PT) – Home Health
Full-Time Baylor | Friday–Monday

We're currently seeking a licensed Physical Therapist to join our growing Home Health Team.
What You'll Do
As a Home Health Physical Therapist, you'll help patients regain independence, improve mobility, and safely recover in the comfort of their own homes by:
- Evaluating patients and developing individualized treatment plans based on physician orders.
- Providing skilled physical therapy services to improve strength, mobility, balance, endurance, and functional independence.
- Monitoring patient progress and modifying treatment plans as appropriate.
- Communicating changes in patient condition to the physician and interdisciplinary care team.
- Educating patients and caregivers on home exercise programs, safety techniques, adaptive equipment, and fall prevention.
- Supervising Physical Therapist Assistants (PTAs) in collaboration with the Director of Rehabilitation.
- Completing accurate and timely clinical documentation.
- Participating in case conferences, care coordination, quality improvement activities, and continuing education.
Qualifications
- Graduate of an accredited Physical Therapy program.
- Current Florida Physical Therapist license in good standing.
- Minimum one (1) year of physical therapy experience required.
- Home health experience preferred but not required.
- Current CPR certification.
- Valid Florida driver's license, reliable transportation, and proof of auto insurance.
- Strong communication, time management, and patient education skills.
