Occupational Therapist (OT) Home Health - PRN in Punta Gorda, Florida at Preferred Care Home Health

Preferred Care Home Health is seeking a PRN Occupational Therapist to join our Therapy team and provide quality, patient-centered care in the home setting.

• Evaluate patients’ functional abilities, including daily living and working skills
• Develop individualized treatment plans with measurable goals
• Provide occupational therapy services in accordance with physician orders and the plan of care
• Help patients improve strength, coordination, range of motion, and independence
• Educate patients and families on therapy programs and adaptive equipment
• Communicate with physicians and members of the clinical team
• Complete timely and accurate documentation
• Participate in case conferences and other required agency activities
• Supervise OTAs as applicable

• Graduate of an accredited Occupational Therapy program
• Current, active Florida OT license
• Minimum of 1 year of OT experience
• Home health experience preferred
• Strong clinical, communication, documentation, and organizational skills
• Ability to work independently and manage a community-based caseload
• Compassionate and patient-focused approach to care
