Job Description

Job Title: Continuity of Care (COC) Therapist
Location: Lufkin, TX
Position Number: 1105
Salary: $61,285 annually

About the Role
Burke is seeking a licensed Continuity of Care (COC) Therapist to provide diagnostic and intake assessments for individuals on probation or parole throughout Burke's twelve-county service area. This position is responsible for determining eligibility for Burke services, coordinating referrals to community providers when appropriate, and ensuring individuals receive timely access to behavioral health services.

The COC Therapist also conducts emergency mental health assessments, coordinates scheduling, dispatch, triage, and disposition of assessments and interventions, and works in accordance with established contract requirements, Medicaid standards, and accreditation guidelines. This position reports directly to the TCCOOMI Service Director and requires travel throughout the surrounding counties within Burke's service area.

What You'll Do
- Conduct diagnostic and intake assessments for individuals on probation or parole
- Determine eligibility for Burke services and coordinate referrals to community providers when appropriate
- Perform emergency mental health assessments and crisis evaluations
- Determine the need for crisis resolution services or inpatient hospitalization
- Coordinate scheduling, dispatch, triage, and appropriate disposition of assessments and interventions
- Collaborate with probation, parole, and community partners to support continuity of care
- Maintain accurate and timely clinical documentation
- Ensure compliance with Medicaid, accreditation, and contract requirements
- Travel throughout Burke's service area to provide assessments and support services

What We're Looking For
- Master's degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling from an accredited college or university
- Strong clinical assessment and diagnostic skills
- Excellent communication and organizational abilities
- Ability to work independently and manage multiple priorities
- Knowledge of crisis intervention and behavioral health services
- Valid Texas driver's license

What Helps You Stand Out
- Six (6) months' full-time direct care experience in a mental healthcare setting
- Six (6) months' direct care experience in mental health screening and assessment
- Six (6) months' direct care experience conducting mental health assessments with both adult and child/adolescent populations

Work Schedule
- Monday-Friday
- Generally 8:00 AM - 5:00 PM
- After-hours work may be required to meet job responsibilities

Licenses & Certifications

Required:
- Licensed Professional Counselor (LPC), Licensed Marriage & Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), or Licensed Psychologist with the State of Texas
- Valid Texas driver's license

Physical Requirements
- Visual and auditory skills sufficient to evaluate and monitor client needs
- Ability to bend, reach, and lift up to 50 pounds while assisting clients and carrying charts or equipment
- Ability to complete SAMA, CPR, First Aid, and Defensive Driving training requirements
- Fine motor skills required for legible writing and documentation
- Ability to drive for business-related travel throughout the service area
- Driving record must be insurable by Burke's insurance administrator
