Job Description
POSITION SUMMARY: Responsible for developing and executing initiatives to decrease patient readmission. This job will have an Inpatient and outpatient component. Assist with completion of Readmission Assessment of current readmitted patients and follow up of patients with high-risk readmission score. Incumbent will also provide interventions/ resources identified to remove barriers as well as education and coordination of care. Facilitates and participate in regular Readmission meetings, organizational/system initiatives pertaining to readmissions, and work with quality/analytics on data collection and staying abreast of organizational goals and implementation and completion of any action plans pertaining to readmissions.
EDUCATION, CREDENTIALS, TRAINING and EXPERIENCE:
Minimum Required: Graduate of an accredited school of professional nursing, with current license to practice as a Registered Nurse in North Carolina.
Thorough knowledge of nursing care process from admission through discharge; gained through several years of clinical experience as an RN.
Minimum of 2 years experience as an RN and 2 years’ experience and as a Hospital Care Manager.
Preferred: Baccalaureate Degree in Nursing and 4 years previous experience as a Care Manager. Knowledge of readmissions and discharge planning methods and procedures. Understanding of clinical pathways and disease management models.
ADDITIONAL SPECIALIZED KNOWLEDGE/SKILLS: Incumbent must have knowledge regarding readmissions, discharge planning, and community resources.
