Job Description

Advanced Practice Nurse-Critical Care

Position Purpose: Provide clinical services as an advanced practice nurse as part of the SAH Hospitalist service. Perform services consistent with advance practice nursing license for critical care patients. Essential Functions
  • Works with the Hospitalist Group to ensure CPOE History/Physicals and Holding Notes are entered on all assigned patients within 24 hours.
  • Ensures Medication Reconciliations are completed for assigned patients.
  • Performs assessments for all assigned patients as discussed in morning huddles with the Hospitalist team. Responds in person to evaluate changes in patient condition and reports to Hospitalist when critical issues arise.
  • Addresses urgent medical issues with the Hospitalist or primary care physician as needed for abnormal lab values, change in clinical status or transfer to a higher level of care.
  • Works with the assigned teams to improve clinical care, quality outcomes for adult medicine patients through collaboration with Hospitalists and key staff by participating in strategic initiatives.
  • Communicates with attending physicians, hospitalists and families about the plan of care.
  • Performs review of Action List in CPOE to signs all verbal orders as required by The Joint Commission.
  • Works in conjunction with the Hospitalist and case management to facilitate the continuum of care post discharge including completion of the discharge summaries.
  • Completes and signs all medical records as required.

Qualifications:
  • Excellent interpersonal, organizational and communication skills.
  • Advanced nursing degree and proof of active state medical license.
  • Maintains a high degree of professionalism with customers and peers.
  • Strong initiative and medical judgment and decision making skills.
  • Ability to prioritize time sensitive medical assignments.
  • Ability to access, understand and explain physician referral patterns.
  • Ability to organize multiple tasks simultaneously.
