About Senior Care Therapy
Senior Care Therapy is a leading multi-state behavioral health organization dedicated to delivering high-quality mental health and therapeutic services to older adults in senior living communities. We partner with skilled nursing facilities, assisted living communities, and continuing care retirement communities to provide compassionate, evidence-based care that improves quality of life for the residents we serve.
Our team of licensed clinicians, administrative professionals, and operational leaders share a commitment to clinical excellence, operational integrity, and meaningful impact. As we continue to grow, we are seeking talented individuals who are passionate about serving the senior population and want to be part of an organization that puts people first.
About the Role
The Clinical Account Liaison (CAL) is Senior Care Therapy’s embedded clinical liaison within an assigned portfolio of approximately 20 skilled nursing facility (SNF) partners. The CAL ensures that residents who would benefit from psychiatric and psychological services are identified, appropriately referred, and connected to SCT’s clinical team in a timely, medically necessary manner.
Working side-by-side with Directors of Nursing (DON), Social Services Directors, MDS Coordinators, consultant pharmacists, unit managers, and other members of the interdisciplinary team (IDT), the CAL strengthens the clinical fabric of each facility partnership, supports appropriate psychotropic medication management, and serves as a continuous education and clinical resource on SCT services. The CAL deepens and stewards SCT’s existing facility partnerships through clinical credibility, education, and embedded relationship work.
What You’ll Do:
Clinical Liaison & Account Stewardship
• Serve as the embedded, recognizable clinical point of contact for an assigned portfolio of approximately 20 SNF partners.
• Build and maintain trusted, peer-level working relationships with DONs, Administrators, Social Services Directors, MDS Coordinators, consultant pharmacists, ADONs, unit managers, and other IDT members.
• Function as a clinical resource and educator on SCT’s services, clinical scope, and referral pathways for facility staff at every level.
Clinical Identification & Medically Necessary Referrals
• Partner with facility clinical staff to identify residents with unmet psychiatric and psychological needs.
• Facilitate medically appropriate referrals into SCT services for new admissions and existing residents with changes in condition, behavior, mood, cognition, or medication regimen.
• Collaborate with consultant pharmacists and prescribers to support appropriate psychotropic medication management, including gradual dose reductions (GDRs) and antipsychotic use aligned with CMS guidance.
• Help facility teams recognize clinical trigger events that warrant psychiatric or psychological evaluation — behavior incidents, new psychotropic orders, schizophrenia diagnostic validation, depression and anxiety presentations, and changes in cognition or function.
Staff Education & In-Servicing
• Conduct ongoing in-services and education for nursing, social services, and clinical leadership on SCT’s services, clinical scope, and referral process.
• Place, maintain, and regularly refresh SCT reference binders at every nursing workstation in each assigned facility — including current contact cards, on-call numbers, referral pathways, and concise service summaries.
• Provide just-in-time coaching to nursing and social services staff on identifying behavioral health needs and initiating appropriate referrals.
Quality, Compliance & Clinical Outcomes
• Support facility partners in CMS quality measure performance related to behavioral health, including psychotropic use, depression, and schizophrenia diagnostic accuracy.
• Participate in psychotropic and behavior committee meetings; contribute clinical input on resident cases within scope of license.
• Operate consistently within HIPAA, facility credentialing requirements, and SCT clinical and compliance standards.
• Escalate clinical or operational concerns to SCT clinical leadership and regional partners in a timely and structured manner.
Account Health & Clinical Continuity
• Monitor facility-level utilization and clinical engagement across the portfolio; flag early indicators of disengagement or attrition risk.
• Coordinate with SCT clinicians (psychiatrists, NPs, psychologists, LPCs, LCSWs) on scheduling, coverage logistics, and facility-specific workflows.
• Maintain accurate visit, education, and engagement records in SCT’s CRM and operational systems.
What You’ll Bring:
Required
• Active, unrestricted Licensed Practical Nurse (LPN) license in state(s) of assigned territory or Active, unrestricted Medical Social Worker (MSW) in state(s) of assigned territory.
• Minimum three (3) years of direct clinical experience in a skilled nursing facility (SNF) or long-term care environment.
• Working knowledge of MDS, care planning, IDT workflows, psychotropic medication management, and CMS quality measures.
• Strong clinical fluency in behavioral health presentations common in the geriatric SNF population — including dementia-related behaviors, depression, anxiety, schizophrenia, bipolar disorder, and delirium.
• Demonstrated ability to build credible, durable working relationships with DONs, social workers, consultant pharmacists, and facility leadership.
• Valid driver’s license, reliable transportation, and ability to travel up to 100% within an assigned regional territory.
Preferred
• Prior MDS Coordinator, Unit Manager, ADON, Staff Development Coordinator, or Clinical Educator background.
• Experience with behavioral health, psychiatric consult services, or geropsychiatry within the SNF setting.
• Familiarity with PointClickCare (PCC), MatrixCare, or other long-term care EMRs.
What We Offer:
• Competitive base salary commensurate with licensure and experience, $65,000–$80,000
• Rewarding experiences working with the senior population
• Flexible daytime hours with autonomy
• Medical, Dental, Vision, Life Insurance, Short-Term Disability, Long-Term Disability, and Ancillary Benefits
• 401(k) plan with up to 3% company match
• PTO & Holiday
