Position Overview We are seeking a skilled and compassionate PRN Speech Therapist (SLP) to provide in-home speech therapy services to adult and geriatric patients. This role offers flexibility, autonomy, and the opportunity to make a meaningful impact in patients’ lives.

Responsibilities

Perform in-home speech, language, cognitive, and swallowing evaluations

Develop and implement individualized treatment plans per physician orders

Provide skilled speech therapy interventions

Educate patients and caregivers on treatment techniques and safety

Monitor progress and adjust care plans as needed

Complete accurate and timely clinical documentation

Collaborate with interdisciplinary care teams

Maintain compliance with HIPAA, Medicare, and agency standards

Qualifications

Graduate of an accredited Speech-Language Pathology program

Active Virginia SLP license required

Minimum 2 years of clinical experience preferred (home health or SNF a plus)

Strong skills with adult and geriatric populations

EMR experience required (Kinnser/WellSky or Homecare Homebase preferred)

OASIS experience preferred

Valid driver’s license and reliable transportation
