🎯 About the Role

We’re looking for a dedicated Clinical Dietitian to join our team and help us continue delivering the level of care our patients and families deserve. In this role, you’ll be a vital part of our clinical team, ensuring excellent service, collaboration, and patient outcomes in a fast-paced healthcare environment.

🔍 What You’ll Do

• Conducts thorough nutrition assessments and re-assessments, identifies appropriate nutrition diagnoses and interventions to address these diagnoses and monitors and evaluates the effectiveness of the interventions, revising as necessary.
• Evaluate the effectiveness of interventions and specific patient-focused outcomes are measured and evaluated. They are revised as appropriate to meet the unique needs of each patient.
• Ensure individualized plans of care are developed and documented.
• Educate patients on a wide range of nutrition topics provided in both individual and group settings
• Continuously seek opportunities to improve processes and support patient and staff satisfaction
• Seek out educational opportunities to stay current and competent in all areas of work responsibilities, including age specific competency.

📋 What You Bring

• Bachelor of Science degree in Dietetics or Foods & Nutrition
• At least one year of relevant work experience
• Registration with The Commission on Dietetic Registration (RD)
• Certification as a Dietitian-Nutritionist (CDN) with New York State Department of Education
• Membership in The Academy of Nutrition & Dietetics and/or The American Association of Diabetes Educators strongly preferred
