Responsibilities:
Perform comprehensive in-home nursing assessments
Deliver skilled nursing care including wound care, medication management, infusions, and more
Monitor patient progress and adjust care as needed
Collaborate with therapists, social workers, and aides
Document visits using EHR with accuracy and timeliness
Participate in case conferences and quality improvement efforts

Qualifications:
Current RN license in DC
2–3 years of nursing experience (home health preferred)
Strong clinical, critical thinking, and assessment skills
Familiarity with home health documentation standards
Tech-savvy with EHR systems
Wound care experience preferred
Reliable transportation to and from the office
CPR certification required
