Do you have what it takes to be a Care Coordinator:
- Completion of a Medical Assisting or LPN education program required
- CMA/RMA or LPN required
- Experience with quality reporting preferred
A day in the life of a HSO Care Coordinator:
- Provide telephone outreach and in-person consultation to members identified as high risk
- Assist in identifying gaps in care
- Collaborate with healthcare team to optimize patient adherence to medical plan of care
- Proactively outreach to members newly diagnosed with a targeted chronic condition
- Educate patient and family members about preventative screenings and identify barriers to care
- Interact with healthcare team
