We've eliminated unnecessary barriers to urgent care through same/next-day appointments, helping seniors avoid emergency rooms and hospitalizations where they risk exposure to additional health concerns.

Pine Park Health is actively building our pipeline of exceptional part-time Nurse Practitioners to deliver care across our SF Bay Area market. While we may not have an opening available right now, we're growing rapidly, and we want to connect with clinicians like you who are interested in joining us when the timing aligns.

Our care model:
This person-centered approach leverages care coordinators and dedicated RNs who handle triage and administrative work, so you focus your clinical expertise where it matters most. You'll practice in comfortable residential settings, develop meaningful patient relationships, and have a real voice in shaping clinical protocols and innovations in our value-based model
When you join Pine Park Health, you'll:
- Provide onsite primary care services to older adults living in Senior Living Communities
- Develop and execute detailed person-centered care plans for all primary care patients
- Work with our Medical Director to identify and develop policies addressing clinical issues, emerging diseases, and practice culture
- Provide monthly Chronic Care Management (CCM) visits and documentation
- Identify ways to collaborate with senior living management to improve patient outcomes and care delivery
- Contribute to our clinical innovation and quality improvement initiatives
- Nurse Practitioner certification with passion for geriatric care (prior geriatric experience preferred)
- Adaptability and resilience in a dynamic healthcare startup environment
- Willingness to provide onsite clinical services across multiple senior living communities
- Dedication to complex care patients and our holistic care model
- Collaborative mindset with appreciation for multidisciplinary teamwork
- Exceptional communication skills for interacting with patients, families, care teams, and community partners
- Tech-savvy approach to embrace digital care tools (EMR, remote monitoring, care coordination)

- Base salary range: $135,000–$160,000 (plus generous quarterly bonuses, equity, and benefits)
