- Schedule: Monday - Friday, 9:00AM - 5:00PM, with holiday and weekend rotations
- Ability to work a flexible schedule that includes evenings, weekends, and on-call as needed to meet the community staffing and resident care needs.
What You'll Do:
- Coordinate day-to-day delivery of Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), ensuring every resident's individualized care plan is followed
- Provide hands-on care as needed, including medication administration, bathing, grooming, dressing, transferring, and mealtime assistance
- Monitor residents for changes in physical or cognitive condition and promptly report concerns to the Director of Health and Wellness
- Build and manage care staff schedules to meet resident needs, census, and budget requirements
- Support recruiting, interviewing, onboarding, and training of new care team members
- Oversee the medication management program in the Director's absence, including the Six Rights of Medication Pass and accurate documentation
- Coordinate with physicians, pharmacists, and other providers to support quality resident care
- Participate in the manager-on-duty rotation and step in wherever the team needs support
What You'll Need:
- Current HCA or CNA certification with Nurse Delegation (or eligibility to obtain per state requirements)
- High school diploma or GED required; some college or clinical training preferred
- 1–2 years of direct care experience in assisted living, memory care, or a related senior living setting (prior lead or coordination experience preferred)
- CPR/First Aid certification (must have or obtain within 90 days of hire)
- Ability to lift up to 100 lbs. with assistance
