Bring your clinical skills to a setting where relationships matter. We are seeking an LPN who combines strong nursing judgment with compassionate, resident-centered care.

What You'll Do
• Provide nursing care within the scope of LPN practice and community licensure.
• Monitor resident health status, identify changes in condition, and initiate appropriate reporting and follow-up.
• Administer medications and treatments as authorized and maintain accurate documentation.
• Support resident assessments, service plans, care coordination, and clinical documentation as assigned.
• Communicate with residents, families, providers, pharmacies, and other healthcare professionals to support continuity of care.

What We're Looking For
• Current, unrestricted LPN license in the state of employment.
• Strong clinical judgment, communication, documentation, and organizational skills.

Preferred Experience
• Experience in assisted living, memory care, long-term care, or senior living preferred.

Physical & Work Requirements
• Ability to stand, walk, sit, bend, reach, stoop, and perform the physical functions of the position for extended periods as needed.
