Homelessness is the largest social and public health crisis in California. Illumination Health + Home is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IHH currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Role

The Manager of Care Coordination & Case Management is responsible to oversee the day-to-day operations of case management and care coordination and supervises all case managers, within the Recuperative Care Department for Los Angeles county. The primary tasks and responsibilities include supervision of staff, assisting in the monitoring of budgets, developing and implementing policies and procedures. The Manager of Case Management will oversee aspects of client care and will link homeless clients with needed available resources, monitor clients’ progress towards goals and provide comprehensive services empowering clients towards self-sufficiency.

This schedule for this position is Monday - Friday, 9am-5:30pm.
The annual salary range is $75k-78k.

Responsibilities

Supervision

  • Supervise case management staff for daily tasks, case oversight, training, and growth.

  • Coordinate hiring process of new case management staff.

  • Work with Site Managers on all client decisions including exits, extensions, intakes, and other case planning action steps.

  • Coordinates orientation and ongoing in-service education for staff to improve skills and client care

Client Care

  • Provide case consultation and emergency management for case oversight.

  • Provide management support to assist case managers and clients in creating crisis management plans or plans to support client’s progress towards self-sufficiency. Plans can include interventions for sobriety, safety, self-harm contracts, employment, housing retention, increase in income, and child safety.

  • Oversee care coordination in partnership with Program Managers and Site Managers for all programs to ensure client care, effective case planning, and positive outcomes in line with the mission of the organization and contract outcomes.

Documentation

  • Ensure accuracy of data for contract compliance, grant reports and other reporting needs.

  • Assists Program Manager with contract compliance as requested.

Collaboration

  • Attend community meetings and collaborations to build relationships and partnerships.

  • Coordinate resource presentations for case managers to increase knowledge of community resources.

  • Excellent communication and interpersonal skills necessary in order to provide effective leadership for subordinate personnel and to develop and maintain a wide variety of internal and external working relationships.

  • Excellent judgment and effective interaction in challenging situations.

  • Working effectively with a diverse and multicultural client body and staff.

  • Demonstrates the ability to maintain a high level of confidentiality of client information, maintaining confidentiality of work related information and materials.

Preferred Experience/Minimum Qualifications
  • Masters Degree in Social Services and 2 years experience in the Social Services field. (preferred)

  • Bachelor’s degree from an accredited college or university in Social Services or related field.

  • Minimum 2 years of recent progressively responsible administrative management/supervisory experience.

  • At least 1 years of experience working with at risk/homeless individuals (preferred)

  • Previous experience working as Case Manager or similar position

  • Experience in supervision or project management and oversight.

  • Current valid CA driver's license

  • Have a dependable vehicle with automobile insurance

  • Computer literate; ability to use Microsoft Office products and databases such as Help Management Information Systems (HMIS).

  • Ability to use Google products.

  • Bilingual in Spanish and English is helpful, highly desirable
