SERVICE COORDINATOR I – HSH/ICMS

JOB TITLE: Service Coordinator I - HSH/ICMS

REPORTS TO: Program Manager

HOURS: 40 hours/week

CLASSIFICATION: Non-Exempt

SUMMARY: The Service Coordinator I (SCI) - HSH/ICMS is a member of the Intensive Case Management Services (ICMS) team. The SCI will engage in the services funded by the Los Angeles County Department of Homeless Services & Housing toward the goal of improving independent living skills and housing retention for individuals living in Step Up’s permanent supportive housing (PSH) programs and in community housing. This position will provide case management and linkage, supportive services, and housing retention services to members utilizing project-based and tenant-based housing in Santa Monica. This position provides program coordination and support to Step Up’s building tenants, property management staff, and Program Manager as determined by the requirements and needs of the program.

DUTIES: The following reflects essential functions for this job but does not restrict other tasks, which may be assigned:

Direct Service and Support

· Participates in HSH and City of Santa Monica referral processes, ensuring applicants will be screened for eligibility for housing opportunities.

· Coordinates lease up activities with the Tenant Manager. Meets applicants together to provide orientation that includes explanations of different roles and how to access services.

· Responsible for lease up by preparing applications and meeting and coordinating with referral to submit application and interview with property management and/or housing authority.

· Performs an intake with all tenants during the first week of move-in.

· Develops initial and quarterly individualized service plans for each tenant which guides the services delivered.

· Meets face-to-face with each tenant weekly and monthly; increases frequency when needed.

· Responsible for tracking, charting, and documentation for all services, file maintenance, and daily logging. Processes paperwork in a timely and accurate manner, as outlined in workflows.

· Takes a “hands-on” approach to demonstrating and supporting tenants’ growth in life skills.

· Regularly assesses and documents changes in tenant income, housing needs, mental health needs, physical health needs, and vocational needs.

· Actively participates with tenants to engage in a Good Neighbor Policy of living responsibly both with tenants of the property and neighboring tenants/business owners in the community.

· Completes “Housing Status Change“ form for all tenants moving in and out of units and annual recertifications.

· Accompanies tenants to appointments in the community.

· Facilitates a safe return to their residence upon discharge from acute care facilities.

· Responsible for providing a tenant-driven, on-site supportive services including engagement, group scheduling, outings and activities, crisis intervention, dispute mediation, ADLs, community integration services, resources and referral, and monitoring tenant safety.

· Maintain an active calendar of supportive services, including at least one supportive group per week.

· Organize group outings and recreational events.

· Interface with DMH teams on lease up activities confirming eligibility for applicants.

Documentation and Data Collection

· Utilize CHAMP for entering all tenant updates/encounters.

· Complete initial Bio-Psycho-Social-Assessment (BPS) once and 5x5 and Acuity Index when completing re-assessment every 3, 6, 9, and 12 months.

· Ensure that appropriate quarterly care plans are developed and update the record with action steps created and fulfilled.

· Other databases may be required relative to the PSH project.

· Enter all demographic data into HMIS/Clarity. Includes entries and exits, changes in income, and updates for any demographic domain annually.

· Participate in the lease-up process. Enter vacancies into MyOrg/HMIS and receive referrals through the Coordinated Entry System where applicable.

· Responsible for managing tenant information on the following documents: Tenant Roster and Contact form and Entry-Exit Form. Updates are required as changes occur. Review for accuracy monthly.

· Other duties as assigned.

SKILLS: Experience working with homeless and chronically mentally ill population, understanding and commitment to psychosocial/Recovery-Oriented philosophy, and ability to work effectively in an interdisciplinary team. Familiarity with and dedication to the Housing

First and Harm Reduction models. Excellent verbal communication skills. Excellent interpersonal and customer service skills. Basic understanding of administrative and clerical procedures and systems. Proficient with Microsoft Office Suite or related software. Strong de-escalation and crisis intervention skills.

QUALIFICATIONS: Minimum of 2 years’ experience working with people who experience homelessness and or are diagnosed with a mental-illness or Social Work/Mental Health related Bachelor's degree and minimum of 1 years' experience working with people who experience homelessness and or who are diagnosed with mental-illness in a community or housed setting necessary. The ability to work as a part of a team is crucial. Knowledge of issues facing program members, (e.g. health, substance abuse, mental health, physical health, domestic abuse, child welfare, resources for undocumented person), excellent written and verbal communication skills, must be able to perform extensive charting, electronic data entry and documentation. Microsoft 365 Office is essential. Must have your own transportation and current California driver's license and insurance with a good driving record at all times during employment.

SUPPLEMENTAL INFORMATION: Work with people experiencing homelessness whether on the streets, in shelters or other places of habitation or services, may present inherent challenges and difficulties such as: exposure to bed bugs or other infestations, unpleasant smells or odors, individuals who may have poor hygiene or unclean homes due to homelessness, mental health symptoms, or poverty. Additionally, in working with our members, employees may encounter instances of profanity, sexually explicit or derogatory language, or verbal or physical expressions of anger and trauma. Very rarely do these situations ever escalate to physical contact, and negative or derogatory communication patterns can often be negotiated successfully. All direct care staff are provided adequate training to develop skills to deescalate crisis situations that may arise and Step Up has established procedural safeguards for all employees to mitigate (but not eliminate) these inherent risks of employment.

PHYSICAL REQUIREMENTS: While performing the duties of this job, the employee is regularly required to sit; use hands and fingers; handle or feel; reach with hands and arms; talk; and hear. The employee is frequently required to walk, balance, stoop, kneel, and/or crouch. The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. Keyboard data entry is required.
