Our Adult primary care practice in Springfield, MA, is seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide nursing and care coordination for patients.
Responsibilities:
- Triage patients through direct contact, or messaging. Determines the clinical priority by assessing the patient's condition, medical history, and present status and then executes appropriate follow-up.
- Handles all prescription refills and call-ins to pharmacies and patients.
- Review patient messages to assess clinical status, coordinate care with providers and schedule appointments according to established protocol.
- Administers immunizations and injections as directed by the primary care provider.
- Communicates laboratory and other test results to the patient as directed by the primary care provider.
- Possesses a willingness to adapt to changing department demands, including assisting clinical staff with procedures, such as EKG, cleaning wounds, obtaining specimens and cultures according to HMG policy.
- Demonstrates willingness to assist co-workers during heightened patient care activity and serves as a resource for clinical staff.
- Performs other duties as assigned.
Qualifications:
Registered Nurse:
Graduate of an accredited program for Registered Nurses; current license as a Registered Nurse within the Commonwealth of Massachusetts; current CPR; two years of experience in a primary care provider's office.
Licensed Practical Nurse:
Graduate of an accredited program for Licensed Practical Nurses; current license as a Licensed Practical Nurse within the Commonwealth of Massachusetts; current CPR; two years of experience in a primary care provider's office.
