Minimum Qualifications: Current Registered Nurse (RN) license in Georgia. Bachelor's degree in Nursing (BSN) or an Associate's degree in Nursing (ADN) with relevant experience. At least one year of nursing experience, preferably in home health or community health settings.

Responsibilities: Conduct comprehensive assessments of patients' health status and needs in their home environment. Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and healthcare teams. Administer medications, treatments, and therapies as prescribed, while monitoring patients for any adverse reactions or changes in condition. Provide education and support to patients and their families regarding health management, disease prevention, and lifestyle modifications. Document all patient interactions, care provided, and changes in condition accurately and in a timely manner.
