About This Position

The Home Care RN plays a crucial role in providing high-quality healthcare services to patients in their homes, ensuring they receive the necessary medical attention and support for their recovery and well-being. Minimum Qualifications:
Current Registered Nurse (RN) license in South Carolina.
Bachelor's degree in Nursing (BSN) or an Associate's degree in Nursing (ADN) with relevant experience.
At least one year of nursing experience, preferably in home health or community health settings.
Spinal cord care experience preferred.

Responsibilities:
Conduct comprehensive assessments of patients' health status and needs in their home environment.
Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and healthcare teams.
Administer medications, treatments, and therapies as prescribed, while monitoring patients for any adverse reactions or changes in condition.
Provide education and support to patients and their families regarding health management, disease prevention, and lifestyle modifications.
Document all patient interactions, care provided, and changes in condition accurately and in a timely manner.

Skills:
The required skills for this position include strong clinical assessment abilities, which are essential for evaluating patients' health needs and developing effective care plans. Excellent communication skills are vital for educating patients and their families, ensuring they understand their health conditions and treatment options.
