Job Description

Definition: One who provides direct care assistance to Consumer/participant in their home under the supervision of Care Crafter Home Care, LLC. Responsibility that service of assistance is delivered in a caring and respectful manner in accordance with relevant Agency policies and standards.

Requirements:

  • Must possess and maintain in accordance with state law, Direct Care Worker certificate and all other applicable certificates.
  • Current physical, PPD, and Hepatitis B Vaccination
  • Current Driver License or state I D and proper Vehicle Insurance Coverage
  • Skills to make Decisions/solve problems, communicate with co-workers and supervisor
  • Maintain Confidentiality and HIPPA privacy.
  • Perform those functions specified for each individual consumer as per care plan.

Responsibilities: Personal Care Services:

  1. Preparation of nutritious meals in accordance with the consumer/participant need and care plan.
  2. Bathing, grooming, and general hygiene.
  3. Transferring in and out of bed and/or wheelchair.
  4. Walking and exercise
  5. Monitoring consumer/participant’s conditions.
  6. Observing and reporting changes in consumer/participant’s condition.
  7. First aid, health and safety.
  8. Scheduling and arranging trips for medical care.
  9. Picking up prescription drugs.
  10. Accompanying consumer/participant on medical appointments.
  11. Accurate documentation of consumer/participant’s liquid intake and toileting.
  12. Assisting consumers with self-administration of medication.
  13. Cleaning the house, changing beds, dusting, mopping, and disposal of trash, washing and ironing of clothes.
  14. Ensure consumer/participant’s safety, security and well- being in accordance with care plan.
  15. Make decisions, solve problems, plus be able to communicate well with co-workers and supervisors.
  16. Provide respite care for families in accordance with care plan.
  17. Observe consumers and their environment and report behavior, physical, cognitive changes and changes in living arrangements to Supervisor.
  18. Observe, receive and obtain information when working in a team environment.
