HealthFinders Collaborative is seeking a mission-driven Licensed Alcohol and Drug Counselor (LADC) to deliver compassionate, culturally responsive substance use services across our clinics, schools, and community settings. This role is part of HFC’s 245G licensed treatment program and serves diverse communities. The LADC provides assessment, counseling, treatment coordination, and recovery-focused support while partnering closely with HFC’s medical, dental, and mental health teams.

Key Responsibilities
Direct Clinical Care (245G Program)
- Provide services in alignment with MN Statute 245G and HFC policies.
- Complete and maintain: Comprehensive Assessments, Treatment Plans, Reviews, Patient Orientation, Discharge summaries, and weekly counseling sessions.
- Deliver individual, group, couples, and family counseling across clinic, school-based, and community sites.
- Facilitate group counseling sessions.
- Provide crisis intervention in collaboration with HFC mental health providers.
- Maintain accurate, timely documentation, billing, and DAANES reporting.
- Participate in weekly supervision, multidisciplinary team meetings, and required trainings.
Coordination, Collaboration, and Engagement
- Work collaboratively with HFC staff, volunteer providers, and community partners.
- Co-manage patient care; communicate with referral sources and outside professionals.
- Participate in outreach, education, and networking activities that promote recovery and wellness.
HealthFinders Staff Duties
- Collect, summarize, and report on data and evaluation metrics in-line with program curriculum, any grant stipulations, as directed by collaborators.
- Attend regular staff meetings and maintain regular office hours at HFC clinic locations.
- Represent HFC’s mission and vision at all times in community.
- Assist other programs and staff, as arranged with your supervisor.
- Current Minnesota LADC or ADC-T license (MN Board of Behavioral Health and Therapy).
- Experience working with individuals with substance use disorders and co-occurring mental health needs.
- Skilled in person-centered, trauma-informed, strengths-based care.
- Familiarity with 245G requirements, documentation standards, and treatment planning.
- Experience providing individual and group counseling with adolescents, adults, and families.
- Ability to work independently, manage competing priorities, and function as part of a multidisciplinary team.
- Experience working in diverse communities preferred (Latinx, Somali, refugee, and underserved populations).
- Strong communication skills and commitment to HFC’s mission and values.
- DHS background check clearance required.
