About This Position
SUMMARY:The Personal Outcomes Manager assists the Residential Director in the operation of the adult Residential Services program which includes support of individuals in assisted living homes. The position has primary responsibility for: Ensuring that the department’s provision of services comply with all applicable regulations, standards and internal procedures. The position is also responsible for assisting the Residential Director with program planning and development, budget and expenditure monitoring, staff recruitment, training and supervision, individual intake process, implementation of Support Plans (SP’s) and interagency coordination. In the absence of the Residential Director, the Personal Outcomes Manager may assume the functions of that position. TASKS, DUTIES AND RESPONSIBILITIES: Monitors implementation of individuals’ Support Plans (SP’s).Oversees Residential Site Supervisors (RSS’) and Residential RN in monitoring individuals’ wellness and medical care.Assists in evaluating program services, designing and implementing program development objectives.Conducts internal audits to ensure:Compliance with Assisted Living Home Licensing requirements.Proper supporting documentation compliance for Medicaid billing including proper ISP (Individual Support Plan) documentation and quality assurance.Compliance with procedures for the documentation of client medical care and medication administration.Ensure appropriate documentation of recipient personal expenditures such as cash logs and POs (Purchase Orders).Monitors purchasing procedure for residences and maintains inventory records.Inspects residential settings and assures necessary maintenance including cleanliness, utilities and security.Inspects residential settings for compliance with licensing regulations, fire, employee and personal safety procedures.Assists in coordinating program intake procedures and assure design and development of individualized services for individuals receiving support.Assists in conducting regular staff meetings.Assists in assuring coordination of residential services with other agency programs.Collaborates with Residential Staffing Manager to meet department and agency objectives.Provides backup support to Residential Site Supervisors, Residential Staffing Manager, or Director of Residential Services as needed or requested.Oversees the training and management of the Residential Program Office Coordinator.Participates in on-call rotation.Attends trainings, in-services, conferences and staff meetings.Makes public presentations regarding agency and residential services.Maintains interagency and community contacts related to residential services.Assists with various agency projects and events.Maintains current knowledge of state laws, regulations, policies and procedures pertinent to residential services. MINIMUM EDUCATION AND EXPERIENCE:Bachelor’s Degree in Social or Behavioral Sciences, Education, Rehabilitation, Business/Health Administration or related field.Three years management experience.Three years supervisory experience in a residential program for individuals with developmental disabilities, preferred.Two years' experience working with individuals experiencing developmental disabilities.Must meet agency insurance criteria for operating agency and personal vehicles on agency business (21 y/o and licensed 3 years). KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:An organized and detail-oriented manager, able to prioritize and delegate tasks effectively to ensure timely completion within a team environment.Able to communicate effectively with all organizational levels, build relationships of trust and enlist the participation of team members.Able to work independently and effectively coordinate tasks to accomplish projects with timeliness and creativity.Thorough understanding of and personal commitment to least restrictive and individualized support for individuals. Demonstrated proficiency in direct care activities and training, and in monitoring the physical, emotional and health needs of the individual receiving support.Thorough knowledge of residential services, applicable policies and regulations, and experience with individualized service planning.Must have good writing skills and computer experience with word processing, spreadsheets and electronic medical records.Must be able to maintain flexible hours, including on-call for evenings and weekendsMust have reliable transportation. LICENSES/CERTIFICATIONS REQUIRED:Alaska Driver’s License**Certification in Adult CPR**Certification in First Aid**Safety Care Certification**QBC CertificationAbility to pass a background check **Must be able to obtain these certifications within 3 months of hire through FRA’s paid training courses. IMMUNIZATION REQUIRED:Annual PPD TestHepatitis B immunization (or) Waiver WORKING CONDITIONS:Sedentary Work: Exerting up to 10 pounds of force occasionally and/or negligible amountof force frequently or constantly to lift, carry, push, pull or otherwise move objects. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.Light Work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force constantly to move objects. PHYSICAL ACTIVITIES:Lifting: Raising objects from a lower to a higher position or moving objects horizontally from position-to-position. Exerting up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.Talking: Expressing or exchanging ideas by means of the spoken word. Those activities which demand detailed or important instructions spoken to other workers accurately, loudly or quickly.Repetitive Motions: Substantial movements (motions) of the wrist, hands, and/or fingers.Driving: Minimum standards required by State Law (including license). ENVIRONMENTAL CONDITIONS:None: The worker is not substantially exposed to adverse environmental conditions (such as typical office or administrative work)
