About the Role

As the Home Care Coordinator (LPN), you will play a vital role in coordinating and overseeing in-home services for Elderhaus participants. Working closely with our interdisciplinary team, caregivers, participants, and families, you will help ensure that each participant receives personalized, high-quality care designed to support their health, safety, and independence.

This position is ideal for an LPN who enjoys relationship-building, problem-solving, and serving as a trusted advocate for older adults.

What You'll Do
  • Coordinate and monitor home care services for PACE participants.
  • Conduct home visits and ongoing participant assessments.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Communicate regularly with participants, families, caregivers, and healthcare providers.
  • Monitor participant status and identify changes in condition that may require intervention.
  • Coordinate caregiver schedules and services to ensure participant needs are met.
  • Provide education, guidance, and support to participants and families.
  • Maintain accurate clinical documentation and regulatory compliance.
  • Participate in care conferences and team meetings focused on achieving positive participant outcomes.
  • Promote participant safety, independence, and quality of life in the home setting.
What We're Looking For
  • Current, unrestricted North Carolina Licensed Practical Nurse (LPN) license.
  • Experience in home health, senior care, long-term care, PACE, case management, or care coordination preferred.
  • Strong assessment, organizational, and critical-thinking skills.
  • Excellent interpersonal and communication abilities.
  • Compassionate, participant-centered approach to care.
  • Ability to manage multiple priorities in a collaborative healthcare environment.
  • Proficiency with electronic health records and basic computer applications.
  • Valid driver's license and reliable transportation.
