Description:
We are seeking an experienced and dependable LPN / Resident Care Coordinator (RCC) to join our clinical leadership team. This role is hands-on and requires someone who is comfortable balancing resident care coordination, medication oversight, and floor support when needed. This is a working nurse role, not a desk-only position.
Key Responsibilities
- Provide direct nursing care to residents in Assisted Living and/or Memory Care
- Administer medications and treatments in accordance with physician orders and state regulations
- Serve as RCC by coordinating resident care plans, assessments, and clinical documentation
- Supervise and support Caregivers/QMAPs during assigned shifts
- Assist on the floor as needed with resident care and staffing support
- Communicate effectively with families, providers, and internal leadership
- Ensure compliance with Colorado Department of Public Health and Environment (CDPHE) regulations
- Participate in audits, surveys, and quality assurance processes
- Respond to clinical concerns, incidents, and emergencies appropriately
Schedule
- Sunday through Thursday
- Tuesday through Saturday

Requirements:Qualifications
- Current Colorado LPN license in good standing (required)
- Experience in Assisted Living, Memory Care, or Long-Term Care preferred
- Strong medication management and clinical judgment skills
- Ability and willingness to assist with hands-on resident care
- Experience supervising or leading care staff strongly preferred
- Strong documentation and communication skills
- Ability to remain calm, organized, and decisive in a fast-paced environment
