JOB DESCRIPTION

HOSPICE CNA

Position Summary:
Provide personal care services such as bathing, grooming, mobility assistance, and support with daily living activities to hospice patients under the supervision of a registered nurse or therapist. Perform care interventions per the plan of care and document all services accurately. Reports to the Hospice Manager.

Key Responsibilities:

  • Deliver personal care as outlined in the patient’s care plan.
  • Assist with hygiene tasks including bathing, oral care, hair care, shaving, skincare, nail care, foot care, and dressing.
  • Support mobility and positioning using assistive devices (e.g., Hoyer lift, transfer board).
  • Help with prescribed exercises under proper training and guidance.
  • Assist with toileting needs and incontinence care.
  • Provide medication reminders for medications self-administered by the patient.
  • Observe, report, and document any changes in the patient’s condition to the supervising nurse.
  • Assist with feeding and meal prep/cleanup as needed.
  • Perform additional tasks as assigned, including taking vital signs, basic infection control, Foley catheter care, emergency procedures, and post-mortem care.
  • Maintain complete and accurate records in accordance with agency policies.
  • Performs other duties as assigned

Qualifications:

  • One year of experience as a nursing assistant (Hospice, Home Health, or SNF preferred).
  • Certified Nursing Assistant (CNA) credential recognized by the Virginia Board of Nursing or equivalent.
  • Ability to read, write, and speak English fluently.
  • Strong verbal and written communication skills.
  • Capable of working independently and meeting deadlines.
  • Must be 18 years of age or older.
  • Physically able to stand/walk for at least 4 hours and climb two flights of stairs.
  • Valid state driver’s license.
  • Satisfactory driving record and proof of current auto insurance.
  • Must complete 12 hours of continuing education, annually.
