Position Summary

Coordinates and delivers primary care in collaboration with the Practice Site Physician by:
• Working within approved scope of practice.
• Obtaining complete medical history and physical data on patients.
• Interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures as needed.
• Performing a complete physical exam and recording findings. Collaborating with physicians in managing the medical needs of patients.
• Order appropriate laboratory and diagnostic procedures.
• Educating and advising patients regarding health and illness prevention. Recommending community resources to meet patient and family needs.
• Support patient access and care coordination through triage, follow-up, and management of urgent situations
Provides clinical and administrative assistance to BMG programs, e.g., education, follow-up, and the like by:
• Participating in health education.
• Maintaining related records and statistical data and preparing statistical reports as needed.
• Participating in interdisciplinary care conferences and assisting with referrals and dissemination of information to appropriate vendors.
• Assisting in the development and coordination of ongoing education and follow-up programs designed to measure client outcomes.
Organizational Responsibilities
Associate complies with the following organizational requirements:
• Attends and participates in department meetings and is accountable for all information shared.
• Completes mandatory education, annual competencies and department specific education within established timeframes.
• Completes annual employee health requirements within established timeframes.
• Maintains license/certification, registration in good standing throughout the fiscal year.
• Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.
• Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patients and self.
• Adheres to regulatory agency requirements, survey process and compliance.
• Complies with established organization and department policies.
• Available to work overtime in addition to working additional or other shifts and schedules when required.
Education/ Licensure / Certification
Required Credential(s):
• Graduate of an accredited Physician Assistant or Nurse Practitioner masters program.
• Current, valid state licensure and appropriate certification (NCCPA or ANCC/AANP) obtained prior to hire date or job transfer date. Verified by local Credentialing Office.
• BLS obtained within 1 Month (30 days) of hire date or job transfer date if required. American Heart Association or American Red Cross accepted.
• Current BLS and ACLS certifications if required.
• Ability to work collaboratively in a fast-paced, team-oriented environment.
Knowledge & Skills:
• Requires a thorough knowledge of practice standards and policies and procedures of BMG and the appropriate professional organization.
• Requires a high level of analytical skills necessary to assess the patient and participate in follow-up care planning.
• Demonstrates interpersonal skills necessary to effectively interact with patient(s), family of patient(s), physicians, other professionals and paraprofessionals.
• Demonstrates high level of communication skills, both oral and written, necessary for effective communication with patient/family, physicians, and referral sources.
Working Environment and Physical Demands:
• Works in a clinical environment requiring physical exertion, frequent changes in job demands, and exposure to biohazards.
• Requires the physical ability and stamina (i.e. to walk/stand for prolonged periods of time, push carts/wheelchairs, lift up to 50 pounds, to position/lift patients at a maximum of 35 pounds without assistance, provide CPR, etc.) to perform the essential functions of the position.
