POSITION OVERVIEW:
The Senior Case Manager will, in coordination with each member’s health home care manager, prepares a personalized Housing Retention Plan. The Case Manager will deliver wrap-around housing retention case management and one-on-one life skills counseling. 254 Virginia St | Buffalo, New York 14201
KEY ESSENTIAL:
- One-on-one life skills and patient activation coaching visits
- Ensure participation from clients with the program, with at least two-time visits from the clients, to complete a two hour one on one case management with each client.
- Participation in selected social support and employment programs at HUB and other area social service providers (e.g., relationship development and peer support programs such as Compeer, basic needs programs accessible through DSS and Community Action Organization).
- Ensure housing maintenance responsibilities, such as apartment cleaning, rent and utility payments (for the portion owed).
- Meet monthly with each individual’s health home care manager to review the individual’s progress.
- Provide quarterly and monthly reports.
- Ensure prompt case notes, documentation, and data quality within AWARDS.
- Responsible for identifying and collaboratively working across programs and organizations to serve the housing needs.
- Assist with Social Services and social security applications.
- Oversee the day-to-day operations for the MRT supportive housing including the housing specialist.
- Ensure policies and procedures are follow for the MRT supportive housing program.
REQUIREMENTS:
- Bachelor’s degree required/ Masters degree preferred
- Minimum of two (2) years’ experience in community based social work experience.
- Must obtain Mandated reporter (2 hours) training/certificate within 30 days of hire.
