Description

Make a Difference in Community Mental Health

Endeavor Health Services is seeking a dedicated and compassionate Behavioral Health Team Case Manager to join our team in partnership with the NFTA. In this role, you will serve as a specialized member of the treatment team, providing direct support to the people we serve by connecting them with essential community-based services outlined in their treatment plans.

This position works closely with mental health counselors to coordinate care, advocate for the needs of the people we serve, and monitor service delivery to ensure treatment and discharge plans are fully implemented. What You’ll Do
  • Meet with the people we serve in their homes and community settings to provide support and service coordination
  • Connect the people we serve with providers and community-based resources identified in their treatment plans
  • Advocate for the needs of the people we serve and help remove barriers to accessing care
  • Collaborate closely with mental health counselors and treatment teams to ensure coordinated service delivery
  • Monitor progress and ensure services outlined in treatment and discharge plans are implemented
  • Identify crisis situations and provide appropriate intervention and support
  • Maintain awareness of prescribed medications and potential side effects
  • Participate in treatment team meetings and contribute to care planning
  • Foster client independence through guidance, encouragement, and skill-building support
What We’re Looking For
  • Bachelor’s degree in Behavioral Science or a related field
  • Minimum of 1 year of experience providing direct services
  • At least 2 years of crisis intervention experience
  • Strong knowledge of behavioral health needs and community resources
  • Ability to work independently and as part of a multidisciplinary team
  • Excellent communication, advocacy, and organizational skills
  • Valid New York State driver’s license and reliable transportation required

Compensation

  • Starting salary: $21.94 – $27.42/hour (based on experience and credentials)
  • $2,500 sign-on bonus
