Reporting to 2 different sites: 65 west 90th street NY and 230 Jay Street, Brooklyn NY


General Responsibilities

JASA is seeking a part-time Case Manager to work two days a week. The Case Manager will follow the standards of the social work profession and the requirements of the funding source.


The Case Manager is responsible for:

● Assessing the social and emotional needs of older adult clients and their families, utilizing prescribed standardized assessment instruments (i.e., PSA, EISEP-PATH); securing, where appropriate, the assistance of other social work and/or other professional consultants.


● Developing and implementing written case plans that provide a full range of social services for the older adult client(s) and their family, including:

o Individual and family counseling and guidance in the resolution of problems of the client(s) and family.

o providing appropriate information to clients and collaterals regarding the normal physical, social, and psychological development of individuals, challenges to functioning presented by conditions of impairment, disease, social stresses, and dysfunction; and

o coping tools and strategies that preserve individual functioning and autonomy.


● Providing specific information about the health, welfare, education, and recreation services available to serve the older client.


● Screening clients for benefits eligibility, assisting with entitlements applications, and providing advocacy.


● Arranging direct services such as transportation, homecare services, meals on wheels, public assistance, Medicare, Medicaid, emergency cash relief, legal aid, protective services, vocational placement, medical and psychiatric examination and therapy, housing, etc.

● Evaluating clients' capacity to manage their affairs, protect themselves from abuse and/or exploitation, and making referrals to Adult Protective Services, as appropriate.


● Referring clients to other community agencies that can meet the client’s needs and fostering cooperative relationships with outside providers.


● Conduct ongoing reassessment of clients’ needs, monitor service efficacy, and make appropriate revisions to the case plan.


● Inform supervisors of crises or circumstances where the worker requires further guidance.


● Conducts group counseling and community education groups.


● Represent and interpret the agency at community conferences and meetings.


● Participate in training and unit meetings.


● Complete required case records, reports, and statistics.


● Perform such other duties as assigned.


Qualifications

● Graduate of an Accredited College or University with a B.S.W./or B.A./B.S. degree, or equivalent.

● Good writing/record-keeping skills.


● Ability to read, write, speak, and understand English.


● Must be computer literate, including the ability to learn and use program-specific software.


● Ability to travel to clients' homes using mass transit and to negotiate any steps and barriers in clients' residences.


● Knowledge of Spanish is helpful.


● Understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values as they relate to serving clients and families.
